Coordinated Entry Project LA HAT Referral Requirements


Dear Providers,


We are reaching out regarding your organization's HMIS project(s) that are currently configured as a Coordinated Entry project type and where LA Assessments have been completed for participating clients.


As part of an ongoing review of Coordinated Entry project configurations, we want to clarify how clients are added to the Permanent Housing Community Queue.


At this time, the LA Housing Assessment Tool completed within a Coordinated Entry project do not, by themselves, place a participant on the Permanent Housing Community Queue. The assessment only covers half of the eligibility requirement; the client must also be enrolled in a qualifying project type recognized by the CES Policy Council Matching guidance. 


Qualifying projects include:

• Emergency Shelter (Entry/Exit)

• Emergency Shelter (Night-by-Night)

• Street Outreach

• Transitional Housing

• Safe Haven

• Housing Navigation (including Non-CoC Housing Navigation)

• Safe Parking

• Safe Sleep

• Access Center

• Navigation Center


If your program is currently configured as a Coordinated Entry project but functions as one of the projects listed above, we would like to review whether the project should be updated to reflect its more appropriate service model.


Please submit a support ticket to the data management department if you believe your project should be configured as one of the qualifying projects listed above, or if you have questions about how your project is currently set up in HMIS. Our team will work with you to review the program and determine whether a configuration update is appropriate.
