Summer Information Blast
Dear Creekside Parents, the new school year is quickly approaching. Check out Start of School Information by clicking the link below:
Schedules were released online July 18th and you may review them until July 24th.
To request a schedule change, students need to submit a request via the schedule change request form online. This form is available on the Creekside website and is provided below for your convenience.

Due to the number of requests, it may take up to 5 business days to process your request. Requests will be processed in the order that they are received.
All forms should have been completed online except for one.

The Student Code of Conduct form must be signed by BOTH the student and the Parent AND returned to Creekside by the first week of school before student IDs are issued.

Students may drop off the competed form during Freshman Day, District Open House, or during Homeroom the first few days of school.

Provided below is the form. Please print, complete, and submit in a timely manner.

Also, provided below are Start of School and PTSO volunteer needs.

***  Volunteers are not the only adults needing school access . ***

Approved School Access ensures the safety of our students, faculty/staff, and our campus. Effective 2015, all visitors and volunteers must have school access. 

  Do you ever???
  • Attend a meeting with Administration or Guidance
  • Attend a PTSO meeting
  • Volunteer time to a Club or Booster on campus
  •  Proctor in the classroom, help in the bookroom, library, etc.
  •  Drive Team members to a game or tournament
  • Volunteer for Special Events (Ticket Sales, Concessions, etc.)
  • Help at a School Sponsored Event (Performance, Camp, Running of the Knights)

If you answered “YES” to any of the above scenarios, it is important to apply for school access online BEFORE your meeting/activity/volunteering. Please go to the school district’s website: http://www.stjohns.k12.fl.us/volunteer/

Please note: You are unable volunteer until your form is cleared in the system. Once cleared, a notification email will be sent to the email address provided on the application. All approvals are valid for three years.

October 2019 PSAT/NMSQT®
      Creekside High School will administer the PSAT to all students in grades 9-11 on Wednesday, October 16, 2019.

The purpose of the test is to give all students an opportunity to experience taking a college readiness test and to identify students who might benefit from advanced coursework. 
Students in 9th and 11th grade must pay a $21 registration fee in order to take the test and receive their scores.

Students will receive an Official Student Guide to the PSAT/ NMSQT® through their English classes. This guide contains valuable information as well as a full-length practice test with answer keys. Please visit www.collegeboard.org/psat for more information.

To pay the registration fee, p lease make checks payable to Creekside High School and write your student’s name, grade level, and “PSAT” in the memo line. Students may submit registration fees to the Testing Coordinator, Mrs. Mackoul, before or after school in room 130.

For your convenience, payment may also be made online though School Pay: https://www.schoolpay.com/pay/for/2019-PSAT-Fee--2100/SdoJhi2   

The last day to pay for registration will be Monday, September 16 th .

All 10th grade students will receive a score report at no charge. Your student’s Score Report for the PSAT/NMSQT® will be available in mid-to-late December from College Board. 
PLEASE NOTE: Students not taking the PSAT on October 16th will be supervised elsewhere on campus.
Per SJCSD Transportation, bus route information will be communicated during the first week of August via HAC and other methods of communication like email and phone messages.
Parents, the nurse will need a Doctor and parent signed medical form for all student’s with chronic health related conditions. If you have questions regarding this requirement, please email Patricia Hardy, BSN, RN at pat.hardy@stjohns.k12.fl.us
The school clinic is located in the main administration building. If your student is unsure of it's location, please tell them to go to the front desk for assistance . Thank you. 
Attendance Policy and Procedure

  • Absence notes are required to be turned in within 2 days of returning back to Creekside in order to be excused.
  • It counts as a day when a student misses 3 or more periods.
  • 5, 10, and 15 day unexcused attendance letters will be generated weekly and Dean Bundshuh will meet with the students who receive these letters.

  • Once a student has missed 5 unexcused days they will receive:

  • the automated phone call,
  • the attendance letter mailed home,
  • meet with the dean and
  • be assigned an after school detention

  • Once a student has missed 10 unexcused days they will receive:

  • the phone call,
  • the letter mailed home,
  • meet with the dean,
  • be assigned an attendance contract and
  • be assigned a Saturday school and/or their parking permit will be taken.

  • Once a student has 15 unexcused absences, they will receive:

  • the phone calls,
  • the letters,
  • meet with the dean,
  • Saturday school.
  • These students may also have their driver’s license revoked from the State DMV.

  • Once a student has missed 15 days of school total (excused and unexcused) they must provide a doctor’s note for the absences to be excused, a parent note will not be accepted.

  • Notes are to be turned in to the front office with 2 days of returning to Creekside. Notes are also required to leave early or check in late. All students must have a planner to check in late or leave early.

  • Please see the Code of Conduct for more information.

  • Attendance will be considered as a requirement for privileges.

PLEASE NOTE: If your student calls you to pick them up because they are sick, it is imperative they understand they must go to the clinic for check out. PLEASE do not arrive and ask us to call them out of class. Instructing your student to go to the clinic prevents class room disturbances and expedites checkout.
All Guidance Counselors will return to school on August 2nd.

Fall Sport Team Tryouts
Students must register and be cleared on Athletic Clearance PRIOR to tryouts. See the Athletics website for instructions. Email the coach for more information.
All freshmen AND new students interested in participating in Athletics( for any sport) during the 2019-20 school year, should attend the Athletics Orientation Event on July 30 th from 6:00 PM until 7:00 PM in the auditorium.
