The holiday season is a time of joy and fellowship. However, it can also be a time of stress and loneliness, especially for people with mental health problems. As the season approaches our mental health team is prepared to meet the increased need of our patients.

One of those patients is Candace. Candace came to CrossOver for treatment for multiple chronic conditions, including diabetes and hypertension. During the new patient intake process, one of our social workers, Abby Dinidid a mental health screening which was positive for depression. Noting this, Abby worked with the patient further and came to realize that Candace had a long-term history of trauma, resulting in anxiety and depression.

Within a short period of time, an entire care team was built around Candace's needs. She is in therapy with a counselor, receives medication management with a psychiatrist, and has a primary care provider to treat her high blood pressure and diabetes. She is also receiving social work assistance to help with housing and food insecurity.
