Crossroads Health Accepts Clients as the Only Certified Community Behavioral Health Clinic in Lake County


Crossroads Health expands mental health services for emphasis on integrated care and care coordination, ease of access to care including 24-hour crisis care and integration of physical health care.

 

Mentor, OH—Crossroads Health has expanded its services and is now a Certified Community Behavioral Health Clinic (CCBHC), the only CCBHC in Lake County. A new service model, CCBHCs are designed to provide a full scope of integrated treatment for individuals living with mental health and substance use treatment needs. CCBHCs offer easy access to behavioral and primary health care, care coordination, and an increased intensity in services to support those with needs in many areas. Crossroads Health’s expanded services include 24/7/365 mobile crisis services, evidence-based services for co-occurring disorders, intensive care coordination services, tailored care for active duty military and veterans, and integration with physical health care. 


Crossroads Health services are available to any individual in need of care, including (but not limited to) people living with serious mental illness or emotional disturbances, addiction, complex physical health needs, recurrent involvement with law enforcement or difficulty managing care independently. Crossroads Health provides care regardless of an individual's ability to pay, caring for those who are underserved, have limited incomes, insured, uninsured or receiving Medicaid or Medicare, and those who are active duty military or veterans.


Crossroads Health’s care coordination involves deliberately organizing client care activities and information among all of the providers concerned with a client's care to achieve safer and more effective care. This means that client needs and preferences are known ahead of time and confidentially communicated at the right time to the right people, and that this information is used to provide safe, appropriate, and effective care to the client. Crossroads Health’s main goal of care coordination is to meet client needs and preferences in the delivery of high-quality, high-value health care with better outcomes.

Crossroads Health is a trauma-informed, evidence- and community-based non-profit behavioral & primary health care center dedicated to providing a comprehensive continuum of services to children, adolescents, young people, adults and seniors throughout all stages of life.
