Patient Insurance Handout

Our patient insurance handout can be found under Administrative Resources on Fulcrum's Provider Portal. This brief handout that you can share with your patient defines the two phases of chiropractic care and explains to your patients which is covered by their health insurance.



Maintenance care phase is not covered through your patient's health insurance. Any financial responsibility they may have for choosing to receive care in the maintenance phase will need to be reviewed with your patient and have a financial agreement signed prior to receiving the care.

Cultural Competency and Disclosure of Ownership Requirement

In accordance with regulatory requirements of the Minnesota Department of Human Services (DHS) and the Centers for Medicare & Medicaid Services (CMS) regulations, all payors, including Fulcrum Health, Inc. (Fulcrum) must collect and maintain information regarding disclosure of ownership and cultural competency from all providers in its network on an annual basis. To learn more about the cultural competency requirement, click here.

 

Fulcrum is requesting that each clinic complete the online form by March 28, 2025.

Fulcrum's Provider Directory Verification

The federal No Surprises Act requires providers and health plans to verify the accuracy of provider contact information every 90 days.


Complete the following steps every 90 days to attest, which fulfills the No Surprises Act requirement: 


Step 1: 

  • View/Update Demographic Information on Fulcrum's Provider Portal.
  • Click on the location that you would like to view/update.
  • Enter your updated information along with the effective date and then click the Submit button for your changes to be saved.


Step 2:

Attest

  • Click on the NSA Attestation button and check the checkbox after reading the attestation and click Submit button.
