Training Overview: Project ECHO (Extension for Community Healthcare Outcomes) is a collaborative model of medical education and care management that empowers clinicians everywhere to provide better care to more people, right where they live. The ECHO model does not actually “provide” care to patients. Instead, it dramatically increases access to specialty treatment in underserved areas by providing front-line clinicians with the knowledge and support they need to manage patients with complex conditions. This forum is a community of learning for complex diabetes management to have brief lectures (15-30 minutes), and case-based discussions about your complex diabetes patients. These sessions will be guided by specialists from the University of Washington and local NCA experts.

Target Audience: Primary care providers and all members of the primary care team in all practice settings who provide front-line care for high-risk individuals with diabetes. Project ECHO® programs result in improved patient retention, better patient outcomes, and enhanced joy in work among providers. This CE/CME activity has been approved for AMA PRA Category 1 Credit™ by Continuing Professional Education, MedStar Health.
