The Developmental Disabilities Administration (DDA)wants to share pertinent information regarding Personal Protective Equipment (PPE). If you are in need of PPE, please see attached form from the Office of Preparedness & Response Emergency Medical Material Request Form  which must be completed and emailed to your local jurisdiction emergency manager.
