REQUIRED FORMS
Here is the quick guide to all required forms! Please note that we need these completed ASAP.
If you received an email saying that you need to complete registration forms, please do that today. If you do not list friends your student wants to be with or shirt sizes, we will make our best guess but cannot make guarantees and cannot make last minute changes. Please provide that information through the link we've sent you if you didn't complete it at registration.
Medical Release Form:
We need a medical release form for every student. If you have already filled this out in the past calendar year, you will not need to do it again. The medical release form can be found at: https://dawson.churchcenter.com/people/forms/191581.
DEFY Waiver (all middle school students):
Middle school students must complete this waiver to participate in the Friday late night activity: CLICK HERE for the DEFY waiver
Medication Form (optional if your student requires medication):
A copy of this form is included at the end of the Info Pages packet. You will print, fill out, and bring this form, along with medication, to your host home on Friday, January 13.
Time Away Form:
If your student needs to leave DNOW at any point, fill out the time away form online at: https://www.dawsonchurch.org/dnowtimeaway.
