NBIB Website Offers Resources to Help e-QIP First Time Users
New or current NIH employees, contractors and affiliates who need help completing the Electronic Questionnaires for Investigations Processing ( e-QIP) can log onto the National Background Investigations Bureau website [ https://nbib.opm.gov/e-qip-background-investigations/ ] where they will find useful guides and resources for filling out their Standard Form (e-QIP questionnaire).*

Users can also log on directly to the e-QIP system from this site via the link, " ENTER E-QIP APPLICATION " where they can complete their e-QIP questionnaire.

The e-QIP questionnaire may take several hours to complete, therefore DPSAC recommends that the individual set aside ample time. DPSAC also advises individuals to not wait until the last minute to complete their e-QIP questionnaire since DPSAC cannot authorize issuance of a PIV Badge and/or authorization for Entry on Duty until the e-QIP questionnaire is complete, and all required materials are received by the deadline referenced in the e-mail.

The individual must complete the final step in the e-QIP questionnaire (" Select 'Release Request/Transmit to Agency' ") to ensure that OPM receives the e-QIP forms. The Division of Personnel Security and Access Control (DPSAC) will contact the individual if additional or clarifying information is needed.
Individuals are encouraged to print their e-QIP archival copy for their own records.

* e-QIP is a web-based automated system that was designed to facilitate the processing of standard investigative forms used when conducting background investigations for federal security, suitability, fitness and credentialing purposes. e-QIP allows the user to electronically enter, update and transmit their personal investigative data over a secure internet connection to a requesting agency.
Q . What information is contained on the PIV Card/RLA Badge? 

A. Each PIV Card or RLA Badge contains the necessary data for the cardholder to be granted access to federal facilities and information systems and assure appropriate levels of security for all applicable federal applications. 

The information on your ID badge will include: 
  •     Your photograph 
  •     Your full name 
  •     Your Operating Division or OPDIV (NIH) 
  •     Badge expiration date 
  •     Badge serial number 
  •     Federal agency smart credential number (which uniquely identifies your OPDIV
and you) 
  •     An authentication key 
  •     Two electronic fingerprints

This information will  NOT  be on your ID badge: 
  •    Social Security Number 
  •    Address 
  •    Phone number 
 
Do NOT lend your ID badge to anyone! Lending your ID badge is prohibited. The issuance of ID badges is based on strict identity proofing and the determination of one's suitability for a specific position classification. To do so is a criminal offense!

Return Your Badge When Leaving NIH
If you plan to leave the employ of NIH, whether you’re an employee, contractor or affiliate, you will need to turn in your HHS ID Badge (or RLA Badge) to your Administrative Officer so that s/he can deactivate your badge in NED. The AO will turn over the deactivated badge to the badging office.

Ad ministrative Officers (AOs) who wish to obtain sponsor authority must complete the sponsor training. To access the training module, click on: Sponsor .

Upon completion, the AO should sign and e-mail a copy of the certificate found at the end of the training module to Alex Salah at: salaha@ors.od.nih.gov . Upon receipt of the certificate, Mr. Salah will authorize the AO as a Sponsor. 

Note: ONLY individuals with an Administrative Officer role in NED are eligible to be HHS ID Badge/PIV Card Sponsors.
