Don't forget to immediately report any incidences to your CCS (ex. seizures, medication errors, hospitalizations, ER visits, injuries, falls, etc.) All incidents involving Persons Served must be reported within an hour of your shift. Additionally, if you are not on shift, but are made aware of the incident, you must also report to your CCS.
Both Parents AND non-family staff are required to report ANY incident to their CCS including normal doctor appointments, injuries, falls, ER visits, hospitalizations, etc.
The following incidents MUST be reported within ONE hour of happening:
- Death of a Person Served
- Any injury to a Person Served that may result in substantial, permanent impairment.
- Unexpected occurrence involving actual or risk of death or serious physical or psychological injury.
- Incidents involving Potential Publicity: Incidents regardless of the category, that might be an interest to the public.
All other incidents must be reported by the end of your scheduled shift.
