Don't forget to bring a Medical Consult Form or obtain an After Visit Summary for all Persons Served medical appointments.


The Gregory Kistler Treatment Center is responsible for maintaining accurate and complete medical records for our Persons Served. Medical Consults or After Visit Summaries can be submitted by placing in Kodie Friend's mailbox at the center, emailed to kfriend@kistlercenter.org, text a copy to Kodie at 479-222-8231, or send a copy with your Kistler Center Care Coordination Specialist (CCS).

IMPORTANT: Human Resources Update

There has been a change to the Holiday Pay Policy.


Going forward, anyone working on a holiday will be paid time and a half!

Anyone not working on a holiday will not be paid for that day.


Our next holiday is Memorial Day, May 25th.

Incident Reporting

Don't forget to immediately report any incidences to your CCS (ex. seizures, medication errors, hospitalizations, ER visits, injuries, falls, etc.) All incidents involving Persons Served must be reported within an hour of your shift. Additionally, if you are not on shift, but are made aware of the incident, you must also report to your CCS.


Both Parents AND non-family staff are required to report ANY incident to their CCS including normal doctor appointments, injuries, falls, ER visits, hospitalizations, etc.


The following incidents MUST be reported within ONE hour of happening:


  • Death of a Person Served
  • Any injury to a Person Served that may result in substantial, permanent impairment.
  • Unexpected occurrence involving actual or risk of death or serious physical or psychological injury.
  • Incidents involving Potential Publicity: Incidents regardless of the category, that might be an interest to the public.


All other incidents must be reported by the end of your scheduled shift.

IMPORTANT: Post Shift Notes

Please click the link below to review specific guidelines for your Post Shift Notes as well as an example note. All Notes from your shift must be entered as soon as possible. It is imperative that your notes are detailed and contain correct information. Notes that do not document progress/lack of progress and thoroughly explain what you did each shift can result in a Person Served having their benefits reduced or lost.

DSP of the Month

Congratulations to Katie Jetton on being chosen as the

April 2026 DSP of the Month!


Katie has been a DSP with the Kistler Center since July 2025!

Criteria to be Selected for DSP of the Month:

  • Consistent, on- time attendance for shifts
  • Good attitude
  • Flexible
  • Notes are consistently turned in on time
  • Consistently clocks in/out
  • Consistently turns in paperwork on time
  • Good communication with CCSs, Person(s) Served, HR
  • Regularly checks email and answers text messages/phone calls
  • Organized
  • Follows Plan of Care for Person Served


To nominate someone for DSP of the Month, please email their first and last name and why you are nominating them to dev@kistlercenter.org.


Nominations for May are due Tuesday, May 26th. Center Staff, DSPs, and Persons Served can all nominate someone for DSP of the Month.


The CES Waiver Leadership Team in partnership with HR and the Executive Director will review the nominations to make a selection each month. All DSPs who are nominated will be given a shout out.
