The lead Performing Provider System (PPS) for Suffolk County under the Delivery System Reform Incentive Payment (DSRIP) program is very pleased to announce the formation of the Suffolk Care Collaborative (SCC). The SCC has resulted from the recent partnership of thousands of health care delivery partners across Suffolk County, NY. Through our partnership, the DSRIP's programs and projects will be designed and initiated with the primary goal of reducing avoidable hospital use by 25% over 5 years. Learn More

Project Advisory Committee (PAC)

Members of the Suffolk PPS are invited to attend the Project Advisory Committee (PAC) Meeting on March 26, 2015. The meeting will be held at the Islandia Marriott Long Island, 3635 Expressway Drive North, Islandia, NY 11749. Registration begins promptly at 8:30am, the meeting will begin at 9:00am, concluding at 12:00pm. Click here to view our program.     


We firmly believe that our vision to become a highly effective, accountable, integrated, patient-centric delivery system has positioned us well to participate meaningfully in the DSRIP Program. Importantly, some of the many goals will include the capacity to optimize patients' self-care abilities, improve access to community-based resources, break down care continuum silos and reduce avoidable hospital admissions and emergency room visits. With your participation, we are confident that these objectives are within reach.


We ask that all participating organizations RSVP in advance to each PAC meeting. Please limit participation to two representatives per organization. RSVP Today! 

Suffolk Care Collaborative Administration

The Office of Population Health (OPH) at Stony Brook Medicine will be the mechanism by which the Suffolk Care Collaborative partners will implement the DSRIP Program.


The OPH, lead by Joseph Lamantia, Chief of Operations for Population Health for Stony Brook Medicine, welcomes the following new hires into the Office - Alyssa Correale, Director, Project Management Office; Amy Solar-Doherty, Project Manager; and Laura Siddons, Project Manager.


The vision of DSRIP is to restructure the health care delivery system by reinvesting in the Medicaid program. The OPH will support the SCC in the design of programs and a care management infrastructure to achieve alignment on system transformation, clinical management and population health.
