FREE CAREGIVER ASSISTANCE
For those caring for individuals with Alzheimer's Disease and Other Dementias grandma-photo-album.jpg

Caring for a loved one with Alzheimer's disease or another dementia can be very challenging and stressful. The Willing Hearts, Helpful Hands Program provides a full range of free services to help ease the many burdens associated with being a caregiver. 

Services Include:
  • In-home consultations
  • Support groups
  • Education and training programs 
  • Respite scholarships for in-home assistance, adult day services or overnights stays
  • Access to volunteer caregiver companions
  • Memory cafes
Services are free and available to anyone caring for a loved one with Alzheimer's disease or other dementias living in Nassau and Suffolk Counties. This initiative is supported (in part) by a grant from the New York State Department of Health and by Parker Jewish Institute for Health Care and Rehabilitation.

"LIVING HEALTHY WITH DIABETES" WORKSHOPS

November was National Diabetes Awareness Month; however, people living with this disease need support year-round.  There is help to best handle this ongoing health condition with this
FREE 6 week workshop on "Living Healthy with Diabetes" starting January 23, 2017.  Meetings will take place Mondays from 1:00 to 3:30 pm at the Middle Country Public Library. Participants will discover new, practical ways to live better and healthier and learn the tools to manage and take control of their health.

NYS DOH DSRIP ANNOUNCEMENTS

"DSRIP DIGEST" SPOTLIGHTS BRONX PPS HOME-BASED ASTHMA INTERVENTION
In this month's "DSRIP Digest," Bronx Partners for Healthy Communities (BPHC) PPS Home-based Asthma Intervention project was featured.  The Bronx is rated the "least healthy" county in NYS and has the highest rates of asthma in the US, including 20% of children and 1 in 11 Medicaid patients.  Read the entire article here.   You can find recent news and upcoming events for DSRIP and previous months DSRIP Digest by clicking here.

CMS APPROVES NY'S REQUEST TO EXTEND  1115 WAIVER
CMS approved NY's request to extend its Medicaid Section 1115 waiver, the Medicaid Redesign Team (MRT) Demonstration, through March 31, 2021.  The 1115 waiver has been renamed from the Partnership Plan to the MRT Demonstration as a reflection of the significant MRT efforts that have improved  and expanded the waiver's purpose.  This approval is the result of significant efforts by the NY and CMS teams, and has been informed by extensive stakeholder input.  The NY waiver renewal letter and Special Terms and Conditions (STCs) are posted on the MRT website  here.

CBO PLANNING GRANT RFA AWARD ANNOUNCEMENT
The New York State Department of Health announced the winning applications in two regions for the Community Based Organization (CBO) Planning Grant RFA #1512160408. The winning application for the New York City region is Arthur Ashe Institute for Urban Health and the winning application for the Rest of State region is S2AY Rural Health Network, Inc. Final award approval is contingent upon the review and approval of the Office of the State Comptroller.   Any questions can be sent to OHIPContracts@health.ny.gov .

CAG PRODUCED LONG-TERM CARE RECOMMENDATION REPORT
The Managed Long-term Care VBP Clinical Advisory Group (CAG) has produced the Long-term Care Recommendation Report, which has been posted for public comment on the DSRIP Website.  Public comment period begins December 12, 2016, and will end on January 9, 2017.  Written comments can be submitted to dsrip@health.ny.gov.  

The CAG Recommendation Reports provides definition and associated quality measures for each VBP arrangement.  The CAG Recommendation Reports inform providers and payers of the details of each arrangement, further supporting their transition to VBP.  Each report represents the final output from each respective CAG for that specific arrangement.  Recommendations from the CAG will be further explored throughout the early phases of VBP implementation.  Based on lessons learned through VBP implementation, each CAG may be reconvened to continue discussions of VBP arrangement definition and quality measures.
