All PAC Members are invited to join the SCC for the March 2016 PAC Meeting. The key theme of this meeting will be Behavioral Health & Primary Care Integrated Care Models.
  
The Committee's mission is to support the goals, strategies, and initiatives among the SCC partners with respect to improving the health status of Medicaid and uninsured communities in Suffolk County.  We invite you to participate and help us promote participation and outreach efforts among the SCC, community and the Medicaid and uninsured patient populations of Suffolk County.

DSRIP BREAKTHROUGH WEBINAR SERIES  
We are excited to share with you the complete DSRIP Breakthrough Webinar Series created by the National Council for Behavioral Health on behalf of the New York State Department of Health for the DSRIP Program. These three pre-recorded webinars are intended to assist PPS networks with the implementation of their selected projects. They are the following:

MEDICAID OPT OUT LETTERS REMINDER
This is a reminder that the Phase II Opt-Out mailing is underway, and was expected to continue through March 15, 2016. Please review the Opt-Out webinar conducted by the Department in October 2015, as well as the updated FAQs on the DSRIP website.   
 
Medicaid members should be directed to call the Medicaid call center for questions they may have on the Opt-Out process. 1-855-329-8850 (toll-free).
  
NYS DOH WEBINAR: ADVANCED PRIMARY CARE MODEL 
New York State Department of Health will be hosting an Advanced Primary Care (APC) model webinar on Monday, April 4th, from 11:00am - 12:00pm.  The purpose of this webinar is to provide an update on the APC model, discuss APC practice requirements and recognition processes, and to answer any questions.
 
The presentation will be given by the Office of Quality and Patient Safety (OQPS), and it is recommended for all PPS Leads, Chief Medical Officers, and DSRIP project staff working on transforming practices to either APC or NCQA 2014 Level 3 PCMH.
 
We ask that you submit any questions you may have in advance of this webinar so they can be incorporated into the presentation. GNYHA DSRIP POST-ACUTE CARE WORKGROUP
SCC Presents at recent Post-Acute Care Workgroup
 
SCC's Clinical Project Manager, Ashley Meskill presented at the Greater New York Hospital Association (GNYHA) Post-Acute Care Workgroup's inaugural meeting. Topics included DSRIP strategies in implementation at Long Term Care facilities. The Suffolk Care Collaborative was joined by Robert Heppenheimer, Project Lead for the SCC DSRIP Project 2bvii and President, Nesconset Nursing & Rehabilitation, and Hilaire Rehab & Nursing, two Skilled Nursing Facilities participating in the SCC INTERACT Program, as well as the New York Presbyterian-Queens PPS.
  
The goal of the GNYHA Post-Acute Care Workgroup will be to bring together PPSs to develop approaches and share best practices for working with post-acute care provider partners to achieve DSRIP program goals. We would like to thank GNYHA for organizing this workgroup and look forward to our future collaboration.
