 This week members of the SCC's Leadership Team presented Population Health Management, Applying Data Analytics & Tools to Implementation Efforts at the Annual DSRIP All-PPS Learning Symposium
ANNUAL DSRIP ALL-PPS LEARNING SYMPOSIUM PRESENTATION BY THE SCC
  
This week the SCC was honored to present as part of a panel discussion entitled Driving Practice Transformation through Performance Measurement, exploring innovative uses of performance measurement data to support practice transformation, including specific data/performance measures, data integration strategies and analytical approaches to promote ongoing population health management. 
 
Kevin Bozza, Sr. Director, Network Development & Performance and Kelli Vasquez, LCSW, Sr. Director, Care Management & Care Coordination presented a program entitled "Population Health Management, Applying Data Analytics & Tools to Implementation Efforts." The presentation included describing the SCC's population health management strategy, which includes six core components of defining the population, identifying care gaps, stratifying risk, engaging patients, managing care, and finally measuring outcomes. Kelli focused the beginning of the presentation around ways in which data is being used to operationalize work around risk stratification of patients and identifying and closing gaps in care through the care management organization with the use of HealtheRegistries. Kevin took the group through the SCC Performance Management strategies, in particular how the SCC has looked to create an algorithm that truly aligns patients to their care providers and subsequently allows performance factor driven funds to flow to the correct Primary Care Provider.  
 
The SCC was joined by panelists from the Staten Island PPS and Albany Medical Center PPS who shared their current strategies for using both state data and clinical data to manage and improve health of patient populations across New York State. Click here to access a copy of the presentation.

EHR & DATA-EXCHANGE INCENTIVE PROGRAMS

NYeC DATA EXCHANGE INCENTIVE PROGRAM
The New York State Department of Health, with support from CMS, has established the Data Exchange Incentive Program to increase HIE adoption across the state for Medicaid Eligible Professionals (EPs).
  
The objective of the NYeC Data Exchange Incentive Program is to increase Clinical Data Exchange contributions from Medicaid Eligible providers. In order to receive funding, all milestones must be completed by September 30th, 2016, click here to learn more.
  
NEW YORK MEDICAID EHR INCENTIVE PROGRAM
Through the NY Medicaid EHR Incentive Program, eligible hospitals (EH) and eligible professionals (EP) in New York who adopt, implement, or upgrade certified EHR technology, and subsequently become meaningful users of the EHR technology, can qualify for financial incentives, click here to learn more.
  
Medicaid providers who are registered as active fee-for-service providers via DOH MEIPASS System, must meet a Medicaid patient encounter threshold as defined by the Department of Health. SUBSTANCE ABUSE AWARENESS & OPIOID OVERDOSE PREVENTION PROGRAM TRAINING CLASS

The Brookhaven Town Supervisor and the Youth Bureau present a free training course provided by the Suffolk County EMS. Topics include what is an opioid overdose, recognizing an overdose and how to administer nasal NARCAN to reverse an opioid overdose. Click here to learn more.

NYS DOH DSRIP ANNOUNCEMENTS
 
GUIDANCE ANNOUNCED ON INTEGRATED CARE SHARED SPACE ARRANGEMENTS
The New York State Department of Health (DOH), the New York State Office of Mental Health (OMH) and the New York State Office of Alcoholism and Substance Abuse (OASAS) have finalized guidance on space arrangements between two or more providers. This document provides guidance to Article 28, Article 31 and Article 32 providers that wish to integrate services through shared space arrangements with other licensed or private medical practices and describes the application process for various types of shared space arrangements. STATEWIDE TRANSFORMATION PROGRAM RFA DEADLINE EXTENDED
The New York State Department of Health, Office of Primary Care and Health Systems Management, has released Addendum #3 to the Statewide Health Care Facility Transformation Program (SCHFTP) RFA. SHCFTP dedicates $195 million in new funding from this year's final state budget to hospitals and other healthcare providers for capital and debt restructuring. 
  
The deadline for the Statewide Health Care Facility Transformation Program (SHCFTP) request for applications (RFA) is extended from September 16 to Wednesday, September 28, at 4 p.m.
  
ADDENDUM TO THE PARTNERSHIP TO EXPAND & ENHANCE NY CONNECTS
The New York State Office for the Aging has released Addendum #1 to the Partnership to Expand and Enhance NY Connects: A Balancing Incentive Program Funding Opportunity for Independent Living Centers (ILC) and/or Community Based Organizations (CBO) RFA originally issued on August 25, 2016. The Addendum #1 extends the application deadline to 9:00 PM on October 1, 2016 and provides an attachment that provides additional space for responses, which can be downloaded from the State Office for Aging's website. In addition, reference to Letters of Support was moved from Section D to Section C.  Reference to specific NY Connects Standards was added to Section D. A Q&A document  for the Funding Opportunity for ILC and/or CBOs RFA has also been posted on the NYSOFA website, click here.  
  
VALUE BASED PAYMENT (VBP) CLINICAL ADVISORY GROUP RECCOMENDATION REPORTS
The VBP Clinical Advisory Groups (CAG) have produced Recommendation Reports for the following VBP arrangements: Chronic Heart Disease, Pulmonary, Diabetes. These CAG reports have been posted for public comment on the DSRIP Website, click here. The public comment period will commence September 14, 2016 and will end on October 14, 2016. Please submit any written comments by October 24th to dsrip@health.ny.gov.  
 
The CAG Recommendation Report provides definition and associated quality measures for each VBP arrangement. The CAG Recommendations Reports inform providers and payers of the details of each arrangement, further supporting their transition to VBP.  Each report represents the final output from each respective CAG for that specific arrangement.  Recommendations from the CAG will be further explored throughout the early phases of VBP implementation.  Based on lessons learned through VBP implementation, each CAG may be reconvened to continue discussions of VBP arrangement definition and quality measures.
