The Long Island Health Collaborative is hosting a Community Based Organization (CBO) Summit in Suffolk County. During the summit, representatives from a diverse group of Community-Based Organizations will share their input on what problems are vital to the people and communities you serve. Data collected during this session will inform the Hospital and County Community Health Assessments. This event is open to all Suffolk County Community-Based Organizations.

NYS DOH DSRIP RESOURCES
NYS DOH DSRIP Program Achievement Values (AVs)  
The New York State (NYS) Department of Health (DOH) has announced the release of the following guide: 'Achievement Value Guide for Performing Provider Systems'. The purpose of this document is to provide additional information on the processes associated with earning Achievement Values (AV) and subsequent payments as part of the NYSDOH Delivery System Reform Incentive Payment (DSRIP) Program. This document covers the definition of what an AV is, how AVs relate to measures and DSRIP payments as well as how Performing Provider Systems can earn AVs and the associated timelines.

PATIENT CENTERED MEDICAL HOME (PCMH) PRACTICE TRANSFORMATIONS UNDERWAY

NCQA 2014 PCMH transformation is underway at Stony Brook Medicine.  The Departments of Family & Community Medicine, General Medicine and Pediatrics began transformation activities last month.  All the practices involved are currently recognized as NCQA 2011 PCMH Level 3 practice sites.  The success of this patient-centered transformation is based on "TEAM" collaboration.  The picture depicts the Family & Community Medicine Transformation Team actively engaged in a practice redesign exercise about creating care teams. There was a great deal of collaboration and excitement during the activity in anticipation of the results.

Integration of Primary Care and Behavioral Health Services Guidance

This week three New York State agencies, the Department of Health, Office of Mental Health and Office of Alcohol and Substance Abuse Services, hosted an Integrated Services webinar.   
 
The purpose of this webinar was to review the Integrated Services requirements, licensure thresholds, DSRIP Project 3ai licensure thresholds, collaborative care models and integrated outpatient services regulations.
 
The objective of DSRIP Project 3ai is to promote the integration of behavioral health and substance use disorder services and primary care for the purpose of ensuring coordination of care.
 
A provider must follow the programmatic requirements of their licensing agency and the prescribed requirements of the IOS Regulations as outlined in the DSRIP Project 3ai Licensure Threshold Guidance, please click here to review the guidance.
