NOVEMBER IS NATIONAL DIABETES AWARENESS MONTH 

According to the American Diabetes Association, almost 30 million children and adults in the U.S. have diabetes, and 86 million Americans ages 20 or older have pre-diabetes -- a condition that puts a person at great risk for developing diabetes. If present trends continue, as many as one in three American adults will have diabetes by 2050.

American Diabetes Association's new campaign   will showcase real-life stories of friends, families and neighbors managing the day-to-day triumphs and challenges of diabetes. Join as they salute the 29 million Americans with diabetes - as well as their loved ones - to raise awareness and to create a sense of urgency about this growing public health crisis. The campaign invites people to submit their own stories to capture the authenticity of those who understand this disease best. The 2017 Open Enrollment period is now open and will continue through January 31, 2017  

HRSA grantees and stakeholders are directly helping Americans learn about the coverage options available in their area and play a unique role in educating their communities about new insurance options available through  Healthcare.gov.  This is important work and it's changing the lives of millions of people. Please help spread the word about the Marketplace and the importance of having health insurance. For those who already have health coverage through the Marketplace, now is the time to help them review that plan and decide if any changes need to be made for 2017. Learn about how you can help spread the word by watching a  video message from HRSA's Acting Administrator . Key Dates:
November 1, 2016: Open Enrollment starts - first day you can enroll, re-enroll, or change your insurance plan through the Health Insurance Marketplace. Coverage can start as soon as January 1, 2017.

December 15, 2016: Last day to enroll or change plans for coverage to start January 1.

January 31, 2017: Last day to enroll or change a 2017 health plan. After this date, you can enroll or change plans only if you qualify for a  Special Enrollment Period through a number of exceptions, like marriage or the birth of a child. 
DSRIP IN CALIFORNIA
Success to Build On
 
In 2010, California's 21 public health care systems (PHS) took a bold and collective step to transform and streamline their complex delivery systems through a federal pay-for-performance initiative. This was the first of its kind in the nation. 

This program differs from New York in that it included 21 of California's public health systems and did not include all healthcare providers in the state.  Each individual health care system's DSRIP plans have included projects aimed at making improvements across the inpatient and outpatient setting in five major areas:
  1. Developing and strengthening their infrastructures
  2. Implementing innovative models of care
  3. Advancing the health of the populations they serve
  4. Continuing to make improvements in quality and patient safety
  5. Improving care coordination for patients with HIV/AIDS

HELP IN THE TREATMENT OF OLDER ADULTS 
SAMHSA-HRSA Center for Intergrated Health Solutions has resources  

The number of adults in the United States age 65 and older is expected to exceed 82 million by 2040. Approximately 16 million of these older adults will have a mental health or substance use condition, and they'll be turning to their primary care providers for care.

Growing Older: Providing Integrated Care for an Aging Population is a new publication from the SAMHSA-HRSA Center for Integrated Health Solutions that offers guidance for primary and behavioral health care providers to be prepared to meet the unique health needs of older adults.

SCC Welcomes New Member of the Team!

Alexandra Kranidis, MPH,  Program Assistant, Project Management Office
 
Alexandra is a recent graduate of the Stony Brook University's Master of Public Health Program with a concentration in Community Health and a graduate certificate in nutrition. Further, Alexandra completed a certification in Public Health in June of 2016. Alexandra graduated from her undergraduate program at Franklin and Marshall College with a Bachelor of Arts, Major in Public Health with a concentration in Biology and minor in History. Prior to joining the SCC, Alexandra was a Senior Research Assistant for the SUNY Research Foundation, where she administered cogitative assessments to 9/11 responders during routine World Trade Center clinical visits. In addition, just prior to joining the SCC, Alexandra worked as a Community Health Worker for Federation of Organizations, contracted by the Stony Brook Children's Keeping Families Healthy Program, where Alexandra provided informal counseling and social support to parents of children with specialty care needs through home visits. Alexandra will continue to work in connection with the Keeping Families Healthy Program through the DSRIP Project 3dii.  
 
NYS DOH DSRIP ANNOUNCEMENTS

BEHAVIORAL HEALTH VBP CLINICAL ADVISORY GROUP (CAG)
Chronic Conditions Recommendation Report
This CAG report has been posted for public comment on the DSRIP Website.T he public comment period commenced on October 28, 2016 and will end on November 28, 2016. 

The CAG Recommendation Report provides definition and associated quality measures for each VBP arrangement.  The CAG Recommendation Reports inform providers and payers of the details of each arrangement, further supporting their transition to VBP.  Each report represents the final output from each respective CAG for that specific arrangement.  Recommendations from the CAG will be further explored throughout the early phases of VBP implementation.  Based on lessons learned through VBP implementation, each CAG may be reconvened to continue discussions of VBP arrangement definition and quality measures.

A POST-ELECTION MESSAGE FROM JASON HELGERSON, MEDICAID DIRECTOR
With the election results, many have raised questions regarding implications for DSRIP.  The following are points to help clarify some concerns:
  • DSRIP is not part of the Affordable Care Act (ACA) and would not be affected by legislative attempts related to the ACA.
  • DSRIP is part of a waiver amendment agreement with CMS, authorized through March 2020 and run by the state government.
  • Health care system reform and Value Based Payment efforts will continue as these are industry drivers for improved quality and sustainability.  Medicare has adopted VBP as part of their program policy and commercial payers are pursuing similar arrangements.
The tremendous efforts by the State and the PPSs to transform the system under DSRIP will continue.   Please keep focused on the DSRIP goals and projects , and maintain the collaborative and positive work that has been accomplished so far.  Proceed with Fact-based Optimism.

ALBANY MEDICAID CENTER HOSPITAL PPS FEATURED IN "DSRIP DIGEST"
Featured in this month's "DSRIP Digest" is Albany Medicaid Center Hospital PPS' Law Enforcement Assisted Diversion (LEAD) project, as well as recent news and upcoming events for DSRIP. DSRIP Digests from previous months can be found here.

NY STATE REDUCED UNINSURED CHILDREN BY 40% IN TWO YEAR SPAN
A new study from the Georgetown University Center for Children and Families shows that New York State reduced the number of uninsured children by nearly 40 percent in two years, the fifth largest decline of uninsured children in the nation. The report shows between 2013 and 2015, the number of children without health insurance in New York dropped by 67,000, achieving a historic high of 97.5 percent insured. The national average of insured children is 95.2 percent. The study attributes Affordable Care Act, which maintained and enhanced existing Medicaid and CHIP coverage for children, to the widespread declines in the number and rate of uninsured children during these two years. PPS PRIMARY CARE PLANS PUBLIC COMMENT
A deliverable under the DSRIP Program for each Performing Provider System (PPS) was the submission of a Primary Care Plan (Plan).  The Plan detailed the PPS' strategy around six fundamentals related to primary care transformation under the DSRIP program.  Plans were due to the Independent Assessor on August 31, 2016.  The Department of Health DSRIP Team reviewed the Plans and provided feedback to each PPS in October, allowing them to resubmit their Plans.   The Primary Care Plans are now posted and are open for public comment until December 21, 2016, and can be found under each individual PPS webpage here
