DSRIP Program Updates Presented by the SCC Project Management Office
PROJECT ADVISORY COMMITTEE MARCH 31, 2016 RECAP
  
We would like to thank all of our partner providers who attended this week's Project Advisory Committee (PAC) meeting held at the Islandia Marriott.
  
PAC members viewed a presentation entitled Integrating Behavioral Health Across the Continuum of Care, presented by Kristie Golden, PhD, Associate Director of Operations, Neurosciences, Neurology, Neurosurgery & Psychiatry, Hospital Administration, Stony Brook Medicine.  
  
Following the presentation, Dr. Kristie Golden moderated a panel discussion on Primary Care - Behavioral Health Integrated Care Practices, panelists included:

ADVANCED PRIMARY CARE MODEL WEBINAR
The New York State Department of Health will be hosting an Advanced Primary Care (APC) model webinar on Monday, April 4th, from 11:00am - 12:00pm.  The purpose of this webinar is to update PPS on the APC model, discuss APC practice requirements and recognition processes, and to answer any questions. The presentation will be given by the Office of Quality and Patient Safety (OQPS), and it is recommended for all PPS Leads, Chief Medical Officers, and DSRIP project staff working on transforming practices to either APC or NCQA 2014 Level 3 PCMH. Large Group of People Celebrating SCC HEALTH FAIR HIGHLIGHTS
SCC Presents at recent Reviving Haiti Health Fair
 
Althea Williams, SCC's Senior Manager for Community & Provider Engagement presented at the annual Reviving Haiti Health Fair in conjunction with YAM Resources held at St. Anne Church in Brentwood, NY. Althea's presentation included educating members of the community on DSRIP and community engagement efforts.  
  
Stony Brook Medicine performed diabetes screenings as well as adolescent mental health screening in collaboration with the School of Nursing students.  We thank all of our community partners involved!
