Dear Parents and Guardians,
 
We look forward to safely welcoming our students back to school this week. Please take a moment to review the following items: 
 
Daily Health Screening
Students must complete the daily health screening Google Form in order to enter school. This form is accessible in each student’s O’Hara email. In order for students to attend school in person, they must provide proof of completion by showing the receipt which is displayed after a student submits the form. The form can be completed after 7:00 PM the night before school. The students may use their phone for this. Please note that only the students coming to school that day need to fill out the form. 
 
Arrival 
Please see the aerial picture of our campus for arrival each day. Students must enter using the East Entrance where their temperature will be taken each day and where they must show proof of their completed Wellness Check form for that day.
Signage
Students are reminded to follow all of the signage in the building. This will be explained more in depth to the students on their first day of school. In addition to social distancing, certain hallways, doors and various portions of the building are one direction and clearly marked throughout the building. 
 
Forms on PowerSchool
 The PowerSchool Public Portal has been updated with our standard AOP Back to School Forms to collect parent eSignatures. All forms must be completed by Friday, September 11. The forms included on the site are:
 
  • Act 195 (Certificate of Individual Request For Loan of Textbooks, Instruction Materials and Equipment)
  • Consent Form For Electronic Communication With Minors
  • Memorandum of Understanding
  • Photo Consent Form
  • Responsible Use Form for Technology
  • Student Release, Waiver of Liability, and Indemnity
  • the bottom of this form includes the handbook acknowledgement, a separate handbook acknowledgement form is not needed
  • Virtual Classroom Video/Audio Recording
 
 
Exposure to Covid-19/Questions
