Missing IDs - Need Your Attention
When reporting chimerism studies for multiple donors/cord blood units there should be one instance with an ID for each donor or cord blood unit for each chimerism test result.
If there is not an ID or birthdate to identify the donor or cord blood unit for the chimerism test results, do not report the results to the CIBMTR. The data is only of value if it can be attributed to a donor or cord blood unit. In FormsNet3, on the Post-TED Form 2450
complete questions 56-74 for each chimerism study by adding an additional instance (click on the yellow plus sign.)

The questions also appear in
Form 2100 R4.0: Post-HCT Follow-Up Data
(Questions 89-107). T
he same rules apply
for reporting the IDs - a new instance for each donor for each chimerism test result for cord blood recipients.
