Missing IDs - Need Your Attention
When reporting chimerism studies for multiple donors/cord blood units there should be one instance with an ID for each donor or cord blood unit for each chimerism test result. If there is not an ID or birthdate to identify the donor or cord blood unit for the chimerism test results, do not report the results to the CIBMTR. The data is only of value if it can be attributed to a donor or cord blood unit. In FormsNet3, on the Post-TED Form 2450 complete questions 56-74 for each chimerism study by adding an additional instance (click on the yellow plus sign.)

The questions also appear in Form 2100 R4.0: Post-HCT Follow-Up Data  (Questions 89-107). T he same rules apply  for reporting the IDs - a new instance for each donor for each chimerism test result for cord blood recipients.

Please only report chimerism data for a multiple donor or cord blood unit when it can be attributed to a specific donor or cord blood unit.

Recipient Update Forms to be Discontinued

Effective October 2, 2017, the NMDP/Be The Match is discontinuing use of the recipient update forms found in FormsNet1 (180 and 183/184). The process for providing recipient updates to donors has been revised following an in-depth review. The changes are designed to eliminate redundancy to transplant centers (TC), strengthen patient confidentiality, and increase the number of updates provided.
Key points:
  • TCs should no longer receive individual requests for recipient updates from NMDP/Be The Match.
  • A survivorship report will automatically be generated from CIBMTR post-transplant recipient data reported on the TED or CRF forms. The report will include:
    • Survival information  - alive or deceased
    • Date of last contact
  • The survivorship report will show available updates indefinitely at the following time points:
    • 100-day survival - available 9 months post-transplant
    • Six-month survival - available 12 months post-transplant
    • One-year survival - available 18 months post-transplant
    • Two-year survival - available 30 months post-transplant
  • DCs will use this report to communicate survivorship information to donors.
For your center not to receive requests from NMDP/Be The Match it is essential that your center report the NMDP RID when completing the CRID Assignment Form (2804).
