Dear Parents,

Summer is flying by, and it's time to get all forms squared away for next school year. Some forms have been sent via email for e-signature. We have a mail slot where you can securely drop your paper forms at any time of the day that is convenient. The mail slot is located next to the door on the side of the administration building that faces the parking lot. You may also submit completed forms via email to mvinfo@trinityschools.org or drop them in the mail.

Here's a complete list of items that are due by August 1. 

• Books and Fees Payment. You can pay online via credit or debit card HERE.

• Annual School Forms. These forms have been emailed to you for e-signature. They include the following: Emergency Contact and Medical Information, Consent and Release, Driver Permission, Photo Permission, and Authorization for Over the Counter Medicine.

• Sports Forms. In order for your child to participate in practice and games, we must have a completed VISAA Sports Physical, signed by a medical practitioner after May 1, 2023, and a signed Concussion Information Sheet on file. The VISAA Sports physical may be signed by your child's pediatrician or by a practitioner at a clinic such as CVS Minute Clinic.

• Forms for New Students. In addition to the above electronic forms, students who are entering Trinity School at Meadow View for the first time must also provide a copy of their birth certificate, and a completed Virginia Health Form, signed by a physician.

• Other Forms as Needed. If your child is prescribed an inhaler, epinephrine, or requires prescription medication during the school day, please submit the following: Inhaler Form (signed by a medical practitioner), Epinephrine Form (signed by a medical practitioner), Prescription Medication Form (signed by a medical practitioner).

Dropping Off Forms

• Our summer office hours are Monday through Friday, 9 am - 2 pm, but we are often working on projects away from the front desk. If you'd like to stop by the office to drop off your forms or ask any questions, give us a call so we can be on the lookout for you.
