Early Access Consideration Deadline Extended for Second Cohort of Illinois House Call Project

Due to the significant interest shown by many of your colleagues, the deadline has been extended to Friday, January 26, for Illinois practices to be considered for exclusive early access, or to nominate another practice, for applying to the second cohort of the ILHCP.

 

The second cohort, launching early this year, will include highly qualified Illinois medical practices (e.g., independent entities, those affiliated with hospital systems, specialty groups, and hospice and palliative care programs). The first cohort, launched in 2022, is well on its way to exceeding its three-year goal. With the addition of the second cohort, the new goal of the ILHCP will be to collectively enroll 6,000 new homebound or home-limited people by 2026.


Each selected practice will designate up to three “Home Centered Care Champions” to lead their work. HCCI will provide the Champions with expertise, guidance, and comprehensive resources, including education, consulting, tools, market analysis, marketing/PR training, and the opportunity to receive capacity-building funding.
