NEW Memory Navigator partnership piloted with Nolia Health at the Memory Hub
Starting January 2026, Nolia Health will be coming on board as the newest provider playing the role of Memory Navigator at the Memory Hub. The Memory Navigator service, initially launched by the Alzheimer’s Association and currently offered by Kin On Health Care Center, offers free 30-minute appointments with a care consultant to discuss topics related to memory loss and caregiving. The care consultant provides information and referrals to community resources such as support groups, respite care, safety assessments, advance directives, home care, housing, legal and financial planning, and more. Staff from Nolia Health will be offering Memory Navigator appointments at the Memory Hub on the 2nd and 4th Tuesday of the month, both on-site and virtually.
Nolia Health is a healthcare organization dedicated to supporting individuals living with neurocognitive disorders such as Alzheimer’s disease and related dementias and their care partners. Through interdisciplinary care navigation, clinical services, and care partner support, Nolia Health seeks to provide wrap-around, culturally responsive, and equitable care that improves quality of life and reduces care partner burden. While Nolia Health is a for-profit company, they are providing Memory Navigator appointments as a benefit to the community and will maintain an unbiased approach without prioritizing their own services.
