Community Assistance Program (CAP) Flex Funds

The Community Assistance Program (CAP) links individuals to a wide array of services that address health-related social needs such as housing, food security, transportation, and utility assistance. 


This program currently uses funding provided by referring partners (such as hospitals and insurance plans). Our team simplifies contracts and billing and streamlines support for vulnerable populations.

HRSN: Program Impact

As part of the state’s recent 1115 Medicaid Waiver, Oregon was approved to offer a new health-related social needs (HRSN) benefit to qualifying Oregon Health Plan (OHP) members. Project Access NOW has been facilitating housing payments in partnership with CareOregon for OHP members who are experiencing certain life transitions, to improve their health during a specific time.


Some examples of life transitions include:


  • Involvement with the child welfare system
  • Recently became eligible for both Medicaid and Medicare (within the past nine months)
  • Experiencing homelessness
  • At risk for homelessness
  • Released from jail, detention center, Oregon Youth Authority facility, prison, or the Oregon State Hospital within the past 12 months

Insurance Enrollment and Payment Support: Program Impact

Our team helps community members enroll in OHP, Marketplace plans, Bridge Plan, and more. We also assist with understanding benefits, scheduling appointments, navigating billing, and accessing financial aid. During Open Enrollment, as well as times of uncertainty and policy change, this support is especially vital.


Not only do we support community members to enroll, we help them understand and use their benefits and act as a trusted guide in navigating the often intimidating health care system.


Additionally, through the Payment Support program, we pay insurance premiums to prevent coverage gaps and make private insurance more affordable.


The open enrollment period for 2026 health coverage is from November 1, 2025, to January 15, 2026, for most states via the national and state marketplaces. To have coverage begin on January 1, 2026, you must enroll by December 15, 2025.
