HMIS Participating Agencies

As the year comes to a close, we want to recognize the Sacramento agencies actively engaged with HMIS. We extend our heartfelt gratitude to each of our HMIS users for the invaluable work you do every day. Your dedication to leveraging HMIS is instrumental in driving solutions to end homelessness. Hope Cooperative has been a proud participant in HMIS since 2015. Hope Cooperative (aka TLCS) is a nonprofit psychosocial rehabilitation agency that empowers individuals to transition from instability to self-sufficiency. Through a combination of permanent supportive housing, mental health services, substance use rehabilitation, and life skills programs, they serve communities in Sacramento and Yolo County. Currently, Hope Cooperative has 27 active HMIS users managing 14 different programs in HMIS.

We are thrilled to welcome Midtown HART as a newly accepted HMIS participating organization! While they currently have just one HMIS user, they are already entering client data for their Day Shelter program. The Midtown HART Day Shelter provides a vital indoor space where individuals experiencing homelessness can find refuge, rest, and an opportunity to recharge. Supported by a dedicated team of 30 volunteers, Midtown HART serves approximately 300 guests weekly, delivering essential care and support to those in need.

Longitudinal Systems Analysis (LSA)

The LSA process is officially underway! This year’s initial submission to HUD continues our positive trend, with fewer errors to resolve than ever before. It’s a testament to our collective efforts toward improved data quality, and we’re thrilled to see the progress. Thank you for your dedication and responsiveness—it truly wouldn’t be possible without each and every one of you.


While we’ve been correcting data errors on the backend for weeks, starting this month, we’ll need your help to address issues that can only be resolved at the program level. Please keep an eye out for any emails from us regarding these requests.


To support you, we’re offering real-time assistance during our Wednesday Support Hour to help answer any questions and guide you through correcting data errors.

New JotForm Software

We've successfully transitioned to JotForm software for all HMIS request processes. This transition has already streamlined operations and improved user experience across the board. Whether you're submitting new agency information, program updates, or HMIS user requests, JotForm offers a more intuitive and efficient submission process. We're excited about this upgrade and remain committed to delivering the highest standards of service and efficiency to our community.

SSF Holiday Closure

During our holiday closure (12/25/2024-1/1/2025), very limited essential staff will reply ad hoc to CAS and HMIS user inquiries. For any housing crisis, please dial 211 then 8. All new HMIS user requests must be submitted to hmis@sacstepsforward.org by 12/20/2024 to make sure the request is captured before office closure.
