Digital Coalition Update
This newsletter is intended to provide you with information regarding updates, and newly posted forms to the South West Primary Care Alliance website.

With many thanks to our willing builders and testers from within the Digital Coalition, we have posted the following PS Suite custom form, Accuro details and interactive PDF files to the form repository.

Digital Coalition featured in Health Quality Ontario 2019 Measuring Up Report
Measuring Up: A yearly report on how Ontario’s health system is performing was recently released and features the work and early success of the Digital Coalition in the South West. Dr. Paul Gill, Dr. Kellie Scott and Executive Director Kim Van Wyk all spoke of their experiences on page 22 of the report here .
Dr. Gill Awarded 2019 OCFP Award of Excellence

In November, Dr. Paul Gill was recognized for his vision and leadership with Partnering for Quality’s Digital Coalition. Dr. Gill credited the work of Digital Coalition members for their role in the forms management initiative saying “there’s a lot of people that go into receiving an award like this” and “all these people deserve a little chip of this.”

Congratulations Dr. Gill and DC members for advising, building, testing, sharing and partnering to improve the referral process for patients in our region! 
Digital Coalition Clinical Advisor Dr. Scott Spearheads Partnership with St. Thomas Elgin General Hospital
Over the past year, St. Thomas Elgin General Hospital has partnered with local primary care providers and the Digital Coalition. STEGH has updated their requisition forms to meet their needs while incorporating feedback to facilitate workflows in primary care and specialist offices outside the hospital that use these requisitions on a daily basis. STEGH now has a suite of updated requisitions that are accessible in a PDF, Telus PS Suite EMR Custom Form and Accuro Form. The Digital Coalition has helped spread these new forms to get them in the hands of providers ordering tests at STEGH, replacing the outdated ones. Most recently, DC Clinical Advisor Dr. Kellie Scott has worked with STEGH Diagnostic Services to develop a new Xray-Ultrasound-BMD Requisition that will be finalized and released in the coming weeks. 
STEGH is one of several hospitals, home care, mental health, and several community support service organizations that are proactively working with the Digital Coalition clinical and advisory leads on the development and digitization of new or updated forms for primary care. Please share the DC Form Request / Response Letter to connect individuals or organizations who may benefit from a similar partnership.

DC FALL WORKSHOP 2019

We received a LOT of great feedback and are working on:

  • How best to get the queue online
  • Coordinating PS Suite and Accuro training (see registration buttons below)
  • Working on a process for sharing "B" forms (those not in the formal DC queue)
  • Working with other regions to investigate how to reduce number overall forms, eg. OBGYN
  • Reached out to OMA for support/endorsement (forms committee: waiting for follow up)
  • Drafting the Digital Coalition Playbook

NEW: SWLHIN First Dose and Iron Sucrose Screener:

After extensive engagement with our stakeholders, we are pleased to move forward with the official launch of our new practice change and minimal standard to support the safe administering of IV First Dose and Iron Sucrose in the Community. (November 1 st ) Read the full memo here

UPDATED: South West Regional Wound Care Program – New Referral Sites

The Diabetic Foot Risk Assessment Form as well as the Diabetic Foot Ulcer Referral form have been updated to include new referral sites listed below. Updated EMR versions are available in the DC EMR Forms Repository posted January 2020

UPDATED: South West LHIN Symptom Response Kit (SRK) Prescription Form

Changes were made to the July 2019 version of the SWLHIN SRK Form to clarify units of measure and other prescription instructions. Updated interactive PDF and EMR versions posted to the SWPCA forms repository within the coming weeks.

UPDATED: LHSC/SJHC Ultrasound Requisition and LHSC Radiologic Consultation Forms

Additions were made to these requisitions including referring provider address information. Updated interactive PDF and EMR versions posted to the SWPCA forms repository.
