This newsletter is intended to provide you with information regarding updates, and newly posted forms to the South West Primary Care Alliance website.

With many thanks to our willing builders and testers from within the Digital Coalition, we have posted the following PS Suite custom form, Accuro details and interactive PDF files to the form repository.

Public Health Ontario COVID-19 Virus Test Requisition

The PS Suite EMR custom form and fillable PDF have been posted the the EMR Resources section of the SWPCA website on March 12, 2020
Accuro users: download “Public Health COVID-19 Virus Test Requisition” under publisher “LisaE” dated 2020-Mar-05 in your Accuro forms repository.



New London Spine Centre Referral
(Victoria Hospital Spine Group and Fanshawe Spine/CBI/Physiatry Locations)

Updated version has been posted to the EMR Resources section of the SWPCA website on March 7, 2020.
Please replace any previous versions with the posted PS Suite EMR custom form or download “London Spine Centre Referral – DC” under publisher “wejerrett” dated 2020-Mar-05 in your Accuro forms repository. 

Please also update your address book to reflect the new fax number: 519-933-4458 .

As always, the above EMR details, PS Suite cfm, and fillable PDF versions are available on the EMR Resources section of the SWPCA website here:  https://www.swpca.ca/EMRResource/ .

Note: The London Spine Centre will also be accepting referrals via the Ocean eReferral network by April 1, 2020 .
