This email is to provide you with an update on our referrals forms management work since the formal launch of the South West Digital Coalition on November 27
th
.
First, I would like to invite you to attend one of two brief teleconferences to review a number of new and updated referral and requisition forms. Please confirm below if you intend on joining one of the following calls:

Referral and requisition form design recommendations have been drafted based on your feedback and are currently being put into practice by several form developers, particularly among our acute care partners. This has allowed our message to spread and has sparked a number of conversations about the challenges being faced in primary care as a result of continual form updates. In fact, we have sped up
a formal communications plan in order to proactively introduce the Digital Coalition and referrals forms management initiative to partners in the acute sector and beyond
.
We are aiming to share communication tools more broadly mid to late February.
We continue to grow membership across the LHIN and have put some training elements in place to ensure “core building” members feel comfortable and confident working together on this important work. We are preparing to release the first batch of forms to come through the DC during the week of February 11
th
, 2019. These forms will be uploaded to the EMR Resources section of
www.swpca.ca
as they become finalized. Any follow-up information will be communicated via email as necessary.
