Total Eclipse of LTC’s Heart!

Many celebrated the April 8 total eclipse, and so did our facilities. Here are just a few photos of the fun activities from our Texas SNFs.

We started an email chain so we could all share if we did anything fun and cute for our residents for the April 8 solar eclipse! I know sometimes we get in the rut of the day-to-day, but moments like today remind me why I love what I do. At Lake Village, we had residents outside with their glasses on and served moon pies! My speech therapist Sydney and I had a photoshoot with some of our patients rocking their glasses.

Celebrating OT Month

Every April, we celebrate Occupational Therapy Month, and this month we would like to highlight Priscilla Martinez, COTA, for the work she does to go above and beyond for her patients at Hunters Pond in San Antonio, Texas.


Priscilla takes pride in her work and can often be found donating crafts to facilitate increased interest in fine motor therapy. She takes time to get to know her patients in order to model a therapy session that targets physical, cognitive, and emotional benefits. She is a team player who steps up to lead Zumba and coordination-driven group therapy sessions. Priscilla recently initiated a Long Term Care Program to address ADL recall and scheduling with cognitively impaired residents through her ADL tracking board. 


Beyond her clinical skills and commitment to the highest quality of patient care as a COTA, Priscilla is a devoted wife, selfless mother, and true friend. Priscilla truly dignifies long-term care in the eyes of the world, and Hunters Pond is both blessed and appreciative to have her on the team.

Huge thanks to our Emerald therapy leaders and their teams, Elyse Matson, Kelly Alvord, Morgan Snyder, Lindsay Burson, and David Gardner, for making this day of learning amazing! Over 60 therapists had the opportunity to learn and grow our dementia care programs. We shared successes, challenges, and best practices for implementing OneClinical practices to improve patient care, survey outcomes, and quality measures. All of our teams have goals and plans to improve care for our residents and outpatients with dementia and to help our fellow team members grow. I am so proud of and grateful to be a part of this Emerald crew! 

Tucson Resource Facility Improvement Project

Back in November, Justin and I walked the property of La Cañada (Tucson) with Donovan Pla, looking at the opportunities for the Resources to share some love and make some improvements. Due to the facility working on some major “formal” improvement projects this year, we chose to share our love outside to help improve the curb appeal. It was an honor to work alongside Justin, Donovan, and all the Tucson Resources, including David Farrington (West Market) and Eric Eller (East Market), and I wanted to share this special day with you. 


On Friday, March 22, 2024, we came together and accomplished clearing and cleaning up a large section of the front part of the property of weeds and overgrowth so that their beautiful desert landscape could start fresh. We also were able to repaint the red curbing along the front of the facility. Thank you, Shelby and Taylor, for owning this part of the project and Tracy, for keeping them safe. 😊Below are some pictures before, during, and after our day of working together in the sun.

In the picturesque landscape of Flagstone South California Market, a gathering of passionate therapists recently converged for an in-person orientation that promised not just knowledge but also inspiration and empowerment. This event wasn't merely a checkbox in their onboarding process; it was a transformative journey that illuminated the intricate dance between nurses and therapists in providing exceptional clinical care.

 

The orientation was a vibrant tapestry woven with threads of insight, engagement, and shared purpose. It peeled back the layers of a typical day in the life of a therapist in a skilled nursing setting, revealing the symbiotic relationship between clinical excellence and the overarching goal of enhancing quality measures and star ratings.

Sunview Respiratory and Rehab Kicks Off “Get Up and Go” Program

The Therapy team at Sunview Respiratory and Rehab kicked off its new OneClinical program to promote Abilities Care and Long Term Care Programming for their residents and promote them getting up every day. The program encourages Therapy, Nursing, and any staff to cheerfully greet the residents each morning by opening their blinds; saying, “It’s time to get up and go!”; and assisting them in getting out of bed for therapy and activities, ultimately helping them “Start Every Day Strong,” as printed on the back of their shirts.  


The Therapy team (Pictured) wears their Get Up And Go shirts every Friday and plans to hand them out to other facility staff they witness participating in the morning greetings and assisting residents to get up.

Legend Greenville's Colorful Approach to Staff Well-Being

That's the heartwarming reality at Legend Greenville in Greenville, Texas, where their "Greenville is Brightening Up Your Rainy Day!" initiative is a shining example of putting staff mental health first. This simple activity has become a beloved tradition, reminding us that even small gestures can make a big difference.


But Legend Greenville isn't the only one looking out for its heroes. Many nursing home staff face demanding schedules and emotional situations that can lead to stress, burnout, and anxiety. Moment of Truth

Jeanne Dunkle, OTR/L, is an Occupational Therapist at Palm Terrace Care Center. When she went to evaluate one of her new patients, she noticed the patient seemed confused and disoriented. The resident motioned to Jeanne several times, looked directly at her, and asked, “Why did they give her a dog?” She then looked to her left and continued, “And they gave her a dog, but nobody gave me a dog!”


On her lunch break, Jeanne went to Target and picked up a cute stuffed dog for this resident. When she brought it to her, she lit up with a giant smile on her face and said, “A DOG,” and she hugged it tight. It made Jeanne feel so good to see her resident so happy.


Thank you, Jeanne, for going out of your way on your own time to do something kind to make this resident happy. We appreciate all you do for our Palm Terrace family.

We are delighted to welcome John Diaz, a highly experienced and dedicated Physical Therapist. With a passion for making a difference in the lives of patients and peers alike, his professional journey began in 2010 as a per diem Physical Therapist at Carmel Mountain. Seeking to stay closer to home and support his growing family, he transitioned to a full-time physical therapist — DOR at Parkside Health and Wellness in 2015, forming a strong partnership with what he calls the "Fab 5" team.


Driven by a deep commitment to continuous learning, he has acquired various certifications that have elevated his expertise and added value to the facilities he supports. These certifications include Resident Assessment Coordinator (RAC-CT), Board Certified Clinical Specialist in Geriatric Physical Therapy (GCS), Advanced Credentialed Exercise Expert for the Aging Adults, and Credentialed Balance and Falls Professional. NET HEALH ERRORS TIPS & TRICKS 

Net Health Errors Explained: What They Mean, How to Avoid Them, and How to Correct Them

Error This record (Caseload) cannot be saved because it was modified by someone else while you were making changes.


At times, a therapist may get an error stating that someone else modified the document they are working on. That “someone else” typically is the therapist themselves. This happens when the computer is moved within the building before saving a document. As a computer is moved through the building, it moves from one Wi-Fi access point to the next Wi-Fi access point, potentially causing a momentary loss of internet. Net Health then sees the current login as a new login for the same user. 


To avoid this error, the therapist simply needs to save their work before moving the computer. This step will prevent this error from occurring. It can occur if there is a flicker in the power or power outage at the building also. Read On...

Purple Is Also the Color of Love

One of our Ambassadors, “Roxanne,” suffered a medical emergency and is currently recovering. As a therapy team, we wanted to send her some love and support by wearing her favorite color, purple. This therapy team is amazing and truly represents “Love One Another.” 

An Afternoon of Service: Keystone Elite and Strong Markets

The DORs from the Keystone Elite and Strong markets joined forces to serve those less fortunate by filling manna packs for the nonprofit Feed My Starving Children. They combined rice and vegetables into sealed bags to be shipped to areas of high famine. Combined with other local volunteers, the teams packed 130 boxes totaling 28,080 meals. These manna packs are delivered all over the world to assist with nutrition in areas with a high risk of malnutrition and famine. 


Following the session, attendants had an opportunity to be a part of a prayer circle for the packs, the volunteers, and the families impacted. It was quite an enriching experience, and the DORs were able to really see the impact of their service. Everyone who attended stated that it was a fulfilling afternoon full of gratitude and service for others.

We also played Jeopardy with Starbucks gift cards for the winning team. The students were very engaged and asked some great questions. The feedback we received was that the students really changed their perspective on LTC/SNF and were very appreciative of our candor, especially regarding productivity and what to look for in a place to work. This was a really fun experience and hopefully something we can continue to provide.

The Universal Language of Music

Jessica Pablo, SLP, has been working with an LTC resident with Alzheimer’s disease who rarely says more than “yes,” “no,” and “okay.” Typically he responds with a smile and a hearty laugh when asked questions. She has been engaging him by using visual aids — such as pictures of various objects — to prompt discussion and facilitate word finding and verbal expression. When presented with a picture of a piano and asked the question, "Who plays the piano?" Jessica received an unexpected but delightful response from our resident: "I do."

 

This response not only demonstrates the power of engaging individuals in activities tailored to their interests and abilities but also highlights the importance of recognizing and celebrating even the smallest communicative breakthroughs. Music is a universal language that transcends linguistic barriers, bringing people together and fostering social connection. He’s been learning a new song almost every two weeks and has been sitting at the piano outside therapy sessions.

30 Years . . . And Counting!

On February 14, 1994, Juan Limones started his first day of work at Victoria Care Center as a CNA. He established himself as a trusted and hardworking CNA loved by residents as well as his fellow employees. In 2019, he transferred to the Rehab department as a rehab aide and has slowly but surely continued to expand his knowledge with regard to helping with scheduling, understanding Net Health and its features, facilitating groups and concurrent therapy, coordinating with the rest of the rehab aides to ensure a smooth workday for the rehab team, and basically being an all-around support to the Rehab team. 


Juan will be the first person to initiate plans to feed the team to boost morale on a particularly hectic work week. His fish tacos are legendary as well as his tacos al pastor. You rarely see him stressed, even after two therapists calling in at 7:30 AM or a concurrent session falling apart at the last minute. According to Juan: “There are no problems, only solutions.”  


On February 14, 2024, we celebrated his 30th year at Victoria Care Center. We surprised him with a decorated workspace as well as a “special bouquet” containing five Coffee Bean gift cards, five scratchers, and 20 super lotto tickets. The team then celebrated his birthday on February 25 with a work potluck that included dishes from the CNAs as well as the Rehab team.  Thank you, Juan, for all you do for the team!
