Notice of Change Overview
In alignment with best practices for database maintenance, assessments that are no longer in use or have been replaced by updated versions will be discontinued. This may include HMIS Assessments that are no longer valid (new versions are available) or no longer needed (the program is no longer operating) or not utilized within 12 months.
Keeping outdated assessments active can:
• Delay the time for participants to access services;
• Create an opportunity for reporting errors; and
• Create confusion; and
• Introduce non-reportable data.
Changes:
All Assessments listed on the link below in the Assessments to Discontinue List below will be deactivated for all HMIS users by June 16, 2026.
