The Spring 2021 Charitable Formulary is here. 

Items of note:
  • The updated Charitable Formulary includes 28 new medications on the Primary Access List. A few specific items to highlight include:  
  • Albuterol Sulfate 0.083% solution for inhalation
  • Azithromycin 250mg and 500mg
  • Cetirizine 10mg
  • Ibuprofen 400mg, 600mg, and 800mg.
  • Naproxen 500mg, 550mg, and 375mg controlled release.
  • Primary Access List medications that treat some of the most common disease states seen by the network:
  • Anti-Infectives - 7 medications including:
  • doxycycline, clindamycin, and metronidazole
  • Cardiovascular - 53 medications including:
  • amlodipine, carvedilol, chlorthalidone, telmisartan, and spironolactone
  • Mental Health - 32 medications including:
  • citalopram, fluoxetine, trazodone, venlafaxine, and aripiprazole
  • Diabetes - 11 medications including:
  • metformin 1000mg XR, Humulin (N,R, 70/30), Humalog, and Basaglar KwikPens
  • Gastrointestinal - 7 medications including:
  • esomeprazole, lansoprazole, prochlorperazine, and vancomycin capsule
  • Our Flexible Access Medication List also has new medications or medications that have moved from the Primary Access list (due to criteria requirements).
