As many of you know, COVID-19 vaccinations are being administered by the Shelby County Health Department (SCHD). The SCHD has considered Shelby County Schools (SCS) employees to be a part of the vaccination timeline. As a result, the District has been asked to survey employees about their willingness to receive the COVID-19 vaccination.
The District's role in this initiative is to assist the SCHD in determining the number of vaccines needed. SCHD is fully responsible for the distribution and administration of all phases of the COVID-19 vaccine.
Please complete this survey expressing your interest to receive a vaccine. Additional information will be provided. Please note that all vaccinations are voluntary, and survey responses are confidential. The completion of this survey is not binding to a vaccination.
