The only way to find out is to regularly track your blood pressure. Sign up for Hello Heart and you will receive a heart monitor that connects to an app on your smartphone. Start tracking your heart health more easily, including blood pressure, cholesterol, heart rate, medication and more.


The Hello Heart app can help you regularly track, understand and manage your heart healthall from the privacy of your phone. The program is available at no cost to State Health Plan primary members (ages 18 and older) who have blood pressure readings of 130/80 mmHg or above or who take blood pressure medication.
