Dear Falcons,
 
Board Policy 2905 (COVID-19) requires all students taking classes in person or using services on campus to provide proof of full vaccination against COVID-19. If you plan to take classes or use campus services IN PERSONyou are required to upload your vaccination card into OptimumHQ unless you have an approved medical or religious exemption from Cerritos College. 

Follow these steps in OptimumHQ to upload your vaccination card. 
 
If you are taking 100% online or remote classes, you are not required to submit proof of vaccination. 
 
Getting Blocked 
 
If you do not have an approved Vaccine Record or Exemption in Optimum HQ, you will not be able to attend classes in person or use services on campus. This may impact your attendance and result in your being dropped from in-person courses. 
 
How to Upload Your Vaccine Cards 
 
  • If you are fully vaccinated, upload your vaccine card into OptmumHQ. For a step-by-step guide on how to upload your vaccine card, visit our website. 
  • Verify that your upload was successful by clicking on “My Check-Ins” from the OptimumHQ dashboard. Look for the file name you used to save your vaccination card under vaccine proof.

All vaccine cards will be verified in the state system. Any fraudulent or altered vaccine cards will result in student discipline and being dropped from any enrolled in-person or hybrid courses. 
 
Not Fully Vaccinated Yet? 
 
If you are in process of getting vaccinated, but will not be fully vaccinated by the start of classes on August 15, please complete this form to be placed on a pending list. Upload a copy of your vaccine card to Optimum HQ showing you have received your first dose. Student Health Services will follow up with you to ensure you reach full vaccination. Be sure to follow through with your vaccination doses. 
 
If you do not upload proof of full vaccination by September 16, you could be blocked from using services on campus and taking classes in person. However, online classes and services are available. 
 
Exemptions 
  • To request a medical or religious exemption, please visit our website for forms. 
  • Submit your exemption request by August 5 to allow sufficient time for the request to be considered prior to classes starting on August 15. 
  • Once approved Student Health Services will update your OptimumHQ account with your Exemption approval.  
  • If you are approved for a medical or religious exemption, you will be required to submit weekly negative COVID-19 test results via OptimumHQ. Free testing is available on campus through COVID Clinic.
