What's New?

We hope you all had a great week and have fun plans for the weekend ahead! Dosberg Manor residents have enjoyed the nice spring weather this week, along with many fun activities such as dot painting, yoga, individual exercise, virtual tours, egg painting, and making ants on a log as part of our weekly "Snax and Relax" pastime. We've enjoyed seeing your lovely faces as in-person visitation kicked off this week!

We are excited to share that we are offering the Independent Health Medicare Family Choice Plan to Dosberg Manor and Garden House residents. Rosa Coplon Living Center residents have benefited from this program for years, and because of its success, we are expanding its availability to our assisted living residents.

What is Family Choice?

The Family Choice plan has been providing an extra level of care and support to residents of nursing homes and assisted living facilities in Erie, Niagara, and Genesee counties for over 15 years. The program offers an integrated team of nurse practitioners, physician assistants, medical directors, and social workers who work with residents and their families to address their healthcare needs right where they live.

How Does it Work?

Plan members are assigned a Family Choice Nurse Practitioner (NP) and a Social Worker who work in collaboration with Weinberg Campus’ staff to develop an individualized care plan that addresses the member’s specific health needs, as well as proactively identify potential issues before they become serious. The Family Choice NP keeps in close contact with the resident’s family and primary care provider, ensuring they are involved in the care planning process.

When Can My Loved One Enroll?

Dosberg Manor and Garden House residents have the option to enroll in the Family Choice Independent Health Medicare Plan effective May 1^st. After that, enrollment occurs on a rolling basis and goes into effect the first of every month.

How Can I Learn More?

This specialized program will allow us to continue focusing on what matters most – enhancing the quality of life and quality of care of our residents by making sure they receive the healthcare services they need, when they need it. The Family Choice team serves as an additional resource to our already qualified and devoted staff and will be available 24/7 should any resident need immediate medical attention.

We are thrilled to offer this healthcare opportunity to our assisted living residents and look forward to welcoming the Family Choice team as an extension of our Weinberg Campus family.

In-person visitation is now in session. In preparation for your visit, please review the following updated visitation guidance:

Weekday visiting hours are Monday through Friday, by appointment only, for the following designated one-hour time slots:
9:30 -10:30 a.m.
10:30 - 11:30 a.m.
1:00 - 2:00 p.m.
3:30 - 4:30 p.m.

Evening visiting hours are available by appointment only on Tuesdays for the following one-hour designated time slots:
6:00 - 7:00 p.m.
7:00 - 8:00 p.m.

Weekend hours are available by appointment only on designated weekend days each month, and be scheduled for the following one-hour time slots:
9:30 - 10:30 a.m.
10:30 - 11:30 a.m.
1:00 - 2:00 p.m.

Please arrive 10 minutes before your designated visitation appointment. If there are extenuating circumstances and you will be arriving late, please call Dana Zientara at 716-639-3311 ext. 2505.

Visiting hours are one hour in length.

Please note that you will need to make an appointment for all visits. If you are dropping off an item for a loved on, a visit cannot take place unless one has been made by appointment.

When you arrive, please sign in at the reception desk. Please sign out when you leave.

You must wait for a Dosberg Manor staff member to escort you to your loved one's room.

Visits will be held in resident rooms; designated visitation areas, or designated outside areas (weather permitting).

If your loved one is currently on a 14-day quarantine, they are not eligible for in-person visitation. However, virtual visits are still available.

All visitors will be screened for COVID-19. This will include a temperature check and you will be asked questions about potential exposure to the virus and recent travel. If you have signs or symptoms of COVID-19 and/or have had close contact with someone with COVID-19 infection in the prior 14 days (regardless of your vaccination status), you will not be allowed to have your in-person visit.

Sanitize your hands when you arrive and throughout your visit.

Wear a face mask covering your nose and mouth at all times.

For residents living in a semi-private room, visitors are not allowed to enter the living space of the roommate or the shared bathroom.

Please use designated seating in each room and please do not move seating to ensure 6 feet of social distancing.

If you need to use the restroom, please use the designated VISITOR restroom past the Deli area.

You are only allowed to travel to and from your loved one's room. You are not allowed to travel anywhere else within the unit or the facility at any time.

Up to two individuals will be permitted to visit a resident at any one time.

Visitors lucky enough to be under age 16 must be accompanied by someone 18 or older.
