Dear Valued Patient,

I am writing to let you know that I will be moving to California to be closer to family and friends in May 2019. I have been honored to provide your care during my many years at Midwest Center for Women’s HealthCare and will miss my patients dearly. Although it will be hard to leave, I am happy and comfortable in the knowledge that my patients will be in the capable and caring hands of my wonderful colleagues: Dr. Kathryn Ray, Dr. M. Susan Scanlon, Dr. Iswar Patel, and practitioners Maria Elena Roux and Melissa Estes. They are all exceptional and you can read more about them on our website at www.mcwhc.com.

Until that time, I am available to see patients in our Arlington Heights and Hoffman Estates offices and address your health care needs. If you would like to schedule an appointment, please call 847.884.9800. The last day I will be seeing patients is May 7, 2019. I request that if you are due for your annual gynecological exam after that time, please do not move up the schedule, but instead set up an appointment for your annual exam at your normal time with one of my physician partners. If you have any prescriptions that are usually filled at your annual exam, they should be valid until your next exam. If you need special prescriptions, I would strongly suggest you contact our office (preferably through our patient portal) prior to May 1, 2019. Any requests received after that time cannot be granted by me and my partners will need to see you for an office visit to establish a physician-patient relationship prior to filling these types of prescriptions.

Your records will remain with MCWHC. Again, I want to assure you that there are many fine physicians and practitioners in our Arlington Heights and Hoffman Estates offices that will be happy to continue to provide you with exceptional care. If, however, you decide to see a provider outside of MCWHC, please complete the Medical Records Request form through our patient portal or on the forms tab on our website (www.mcwhc.com).
