Looking Ahead

Immunization, Medical, and Emergency Contact Notes

Student Immunization Requirements
All students
must have a current, valid certified immunization record on file in the nurse’s office in accordance with state requirements. Parents are encouraged to review their child’s vaccination status with their pediatrician.

To be considered valid, the expiration date at the top right of the page must have a future date listed. If the record is expired or if immunizations are not up-to-date, the record is not valid.

If you are unsure whether your child’s immunization record is still valid, please request an updated certified immunization record from your child’s pediatrician before the start of the 2026–2027 school year.  Records may be uploaded here by August 3, 2026.
