IMMUNIZATION STATUS:
Every parent should review their child’s vaccination status with their child’s pediatrician. Every student’s immunization status should be up to date and submitted to the school by August 1, 2025.

Special Note to INCOMING 6th Graders:
Depending on your child’s vaccination history, according to the American Board of Pediatrics, a second varicella vaccine (provides immunity against chicken pox) as well as another Tdap (Tetanus, Diphtheria, and Pertussis) may be recommended.
Please make arrangements with your child’s health care provider to ensure immunization status is up to date prior to August 7, 2025.
Please upload current documents to the immunization tab on the school website prior to the first day of school.

NOTICE PURSUANT TO LA. R.S. § 17:170(E)(2)
In accordance with Act No. 675 (HB 47 of the Louisiana Legislature’s 2024 Regular Session) and La. R.S. § 17:170(E)(2), please be advised of the following: La. R.S. § 17:170(E)(1) provides that “No person attending or seeking to enter any school or facility enumerated in Subsection A of this Section shall be required to comply with the provisions of this Section, including any additional immunization or proof of immunity requirement adopted pursuant to the provisions of this Section, if the person is a distance learner or if the person or his parent or guardian submits either a written statement from a physician stating that the procedure is contraindicated for medical reasons, or a written dissent from the student or his parent or guardian.”
HEALTH AND EMERGENCY FORMS:
All information should be reviewed and updated each summer. If your child has a medical condition that requires medication, please make sure that medication is also listed on the form under medication and the reason the child takes the medication is also listed. Find your child’s health and emergency form in your FACTS Family Portal under Web Forms / Family Demographic Form.
Parents' names and contact information must be included in the Emergency Contact section along with other listed contacts.
Medication Drop Off Dates
The designated medication drop off days are July 23, 2025 from 8 am to 3 pm and August 1, 2025 from 8 am -12 pm. Plan to drop off medication and completed paperwork together. We cannot accept one without the other or incomplete forms. If a picture is needed, please attach a recent photo. All medication for Aftercare should be delivered at this time as well. Please make sure your student's health and emergency form in FACTS is up to date PRIOR to meeting with us.
