Medication Forms and Policies
Student Medication Policy:
- Students are not allowed to have any medication, prescription or non-prescription, in their possession at any time on the school grounds.
- If medication must be administered at school on a regular basis, a Parental Consent Form for Medication Administration for each medication must be completed and signed by the parent and the physician. The physician’s portion of the form must include the child’s name, diagnosis, name of the medication, time/frequency to be administered at school, dosage and length of time to be administered. This form must accompany the medication.
- Prescribed medication must be in an updated container that meets acceptable pharmaceutical standards. The label must include the name of the student to be administered medication, name of medication, strength, and amount and time it is to be administered.
- A new physician’s order is needed at the beginning of each school year. Verbal phone orders from physicians or parents cannot be accepted.
Prior to bringing medication to the school for medication drop off:
- Review paperwork to ensure all sections, including the photo and signatures, are completed.
- Medication and all completed forms must be submitted together in a labeled ziplock bag to be accepted.
- Visit the student's health and emergency form in FACTS Parent Portal to reflect the medication to be administered.
- If your child requires medication during the school day, the same medication must be provided for Before and After Care. Please bring the additional medication during the initial medication drop off. This additional medication must be provided before your child may attend school and/or Before and After Care.
Immunizations - Every student’s immunization status should be updated by August 1, 2026. Parents should review your child(ren)’s vaccination status with your child’s pediatrician.
There are 3 ways to have your child's immunization status "up to date”:

A current, signed, up to date, Louisiana Immunization record should be uploaded on the school website by August 1st, 2026 for EVERY student grades PreK-8th.
The actual hard copy should be titled “State of Louisiana Universal Certificate of Immunization”. This document is obtained from your child's pediatrician's office. Immunization form requirements: current (future expiration date), and should be signed (by the pediatrician's office). Upload the record to the St. George website under Resources/Nurse's Office/Certificate of Immunization Record Submission.
- After discussing your child’s health and vaccination history, sign a dissent and upload to the St. George website under the Immunization link. Mrs. Fontenot will sign it and it will be kept on file. Every year, you should review this and a new letter is required every school year if you choose to continue the same path.
- If your child has a flagged immunization or delayed/staggered shot schedule, have your pediatrician write a letter explaining your child's case and upload that letter to the St. George website under the immunization link. Mrs. Fontenot will sign it and it will be kept on file. A new letter is required every school year.
Special note to INCOMING 6th graders:
As mandated by state law, children 11 years old and entering the 6th grade need to provide proof of immunization against meningococcal disease. According to the American Board of Pediatrics, a second varicella vaccine (provides immunity against chicken pox) as well as another Tdap( tetanus, Diphtheria, and Pertussis) may be recommended depending on your child’s vaccination history.
