Reducing Recidivism Through Collaboration

From uninsured and incarcerated to healthy and employed, Dwight Rice's story can inspire us all.
"For the past few years, Spartanburg Regional Health System and AccessHealth have worked with the local detention center to target inmates with  medical and behavioral health problems. The goal is to reduce hospital emergency room visits and reduce recidivism. And early data suggest it's working."

Spartanburg's Way to Wellville

Health 2.0: Technology for Healthy Communities
Access to care for the uninsured is one of the five core areas Sparanburg's Way to Wellville focuses on impacting to improve the community.

Over 200 innovators across the U.S. submitted applications to Technology for Healthy Communities, a program sponsored by the Robert Wood Johnson Foundation and powered by Health 2.0. AccessHealth Spartanburg was chosen as one of three pilot programs and matched with the high performing health tech innovator, Healthify, a NYC based company with the leading collaborative software solution for healthcare and community services.

With the support of Spartanburg's Way to Wellville and Mary Black Foundation, we will pilot a community resource platform to provide community interventions for social determinants of health at the point of care.
