Reviewer Forms
Dear Select IRB Reviewers,
Pertaining to the E-IRB System Update on August 12, 2020, please note the following:
Effective immediately, the below listed forms are no longer listed under the “Attachments” tab for the assigned IRB reviewer to download as a PDF, complete & save separately from E-IRB, then upload into E-IRB with the appropriate Document Type. When assigned by ORI in the Select Reviewer task window or via the Re-Select Reviewer tool, these forms are now pages within E-IRB to be completed by the Reviewer in the applicable tab of the IRB Reviewer’s task window (see examples in screen shot below):

Consultant Page –
- includes the instructions on what the consultant is being asked to review and comment on under the Finish tab, and
- the Conflict of Interest Statement and Confidentiality Agreement (requires checking box for acknowledgement)
- Prisoner Representative Page
- 2019 Exemption Signature Page (P2)
