All Care’s EPIC Transition

As we provide affordable, comprehensive care to our patients and respond to the ever-changing healthcare system, All Care Health Center is implementing a new electronic health record (EHR) system, Epic.


This nearly statewide collaboration of 11 community health centers is a new, unique national model, providing a centralized, top-of-class EHR solution in the state's rural and urban areas. This project will be managed by INConcertCare (INCC), a sister organization of the Iowa Primary Care Association, in collaboration with OCHIN, a nonprofit leader in equitable healthcare innovation and a trusted solutions partner.  


All Care went live with OCHIN Epic on August 29, 2023. We understand that challenges can accompany change, and we want to assure you that we are committed to making this transition as smooth as possible. Our implementation team is working diligently to ensure a well-planned and coordinated approach that minimizes disruptions and maximizes the benefits for all stakeholders.


We highly value our partnerships and believe the move to OCHIN Epic will enhance All Care Health Center and our integrated network, connect with other systems, share data, make informed decisions, and collaborate with other partners. As we implement our new EHR, we especially appreciate the unwavering support of our partnerships. We look forward to the continued success and growth we will achieve together with the implementation of OCHIN Epic.

MyChart Provides an Ease to Patient Communications

With the launch of Epic comes the ease of MyChart for All Care patients. MyChart is All Care's new patient po,rtal allowing patients to connect with their providers, request appointments, request prescription refills, view test results, and access health records.  


Patients should now register for MyChart by visiting this link or scanning the QR code to access the new system's benefits. Patients who sign up before their next appointment must scroll down to use the "No Activation Code?" option.  


Once signed up, patients will note that the homepage features quick access to information needed most, including upcoming visits, messages, test results, and medications.  


Below the homepage shortcut area is a feature called the Health Feed. This includes the most important alerts and information for patient care, such as messages from their care team and appointment reminders. Action can be taken directly from the Health Feed. For instance, patients can open a message and reply if needed, view details of an upcoming appointment and add them to their calendar, confirm the appointment, and complete check-in tasks or get on a waitlist. Patients with MyChart proxy access for a family member or someone else cared for can include these patients in their alerts on this feed. MyChart allows patients to switch to that person's account for more in-depth details.  


The MyChart website has the same features as the MyChart mobile app. To learn more, check out videos after account creation.  


Patients needing to share COVID-19 vaccination status or test results for travel or an event can use the button at the top to quickly share COVID-19 information via QR codes, printable PDFs, and more.

Thank you 2023 Spirit Award Winners

Every year All Care presents community partners or vendors with the All Care Spirit Award. This award recognizes community partners or vendors who support us by helping us meet our mission. Awardees are both nominated and voted on by All Care employees. Our 2023 award winners are American PCS and Story Street Pantry. 


American PCS believes in reliable solutions, a commitment to exceptional service, and a human-centered approach while having a passion for helping nonprofits. All Care staff are recipients of that great service with American PCS support accessible with a simple email for any of our staff to utilize. 



Story Street Pantry was established in 2020, during the COVID-19 pandemic, to leverage efforts to combat food insecurity. Their mission is to “offer healthy, nutritious food at no cost to people in need while always treating our clients with the highest level of dignity and respect.” All Care began its partnership with Story Street Pantry in 2022, believing that whole-person health includes access to nutritious food. Our staff has loved the opportunity to serve patient needs outside of clinic care.

While All Care Health Center loves to provide integrated healthcare under one roof, the health center knows the importance of providing access throughout the community.

 

New and returning patients of All Care Health Center can receive care at several locations. Most widely known is the main health center at 902 South 6th Street in Council Bluffs. At this location, All Care provides primary medical care, mental health, dental, a patient pharmacy, and x-ray services all under one roof. This location is also the hub for our access services team that helps community members to enroll in health insurance at no cost. The access services team also helps patients to apply for the sliding fee scale and to find other services that help to fill patient gaps.

All also has a second location, the Florence M. Lakin Health Center, which is housed on the Charles E. Lakin Human Services Campus at 1415 Avenue J, in Suite 2. This three exam room clinic has provided neighborhood access to primary medical care and dental care since 2019. Special services are also available at this location through a partnership with MICAH House to care for their guests.


New in the fall of 2022 is the All Care Health Center mobile healthcare clinic. This clinic on wheels boasts two exam rooms and a lab draw area. The mobile clinic is currently providing primary medical care and psychiatric mental healthcare services, including medication management, for Council Bluffs Community School District students and families at Thomas Jefferson High School and team members and families of Tyson Foods. The mobile health clinic is also utilized for specialty clinics such as sports physical and immunization events. 

Some patients prefer to receive healthcare at their own home or work. All Care can complete these visits via telehealth. This service began in the spring of 2020 as the COVID-19 pandemic jolted us into a new normal. Telehealth services are available as a bridge for long-distance patient and provider clinical services, such as diagnosis and monitoring. These services can be performed using a patient computer, laptop, tablet, or telephone. Patients should ask if telehealth will meet their needs when scheduling an appointment.


While these options meet most patient needs, All Care also provides primary medical care and mental health services for New Visions Homeless Services guests. If you know someone currently experiencing the crisis of homelessness, please encourage them to seek healthcare using a homeless voucher. All Care will not deny anyone care based on their inability to pay.
