How to Open the Modalities Discussion with Patients

You already know the conversation needs to happen every year, but it seems there is never enough time to get in depth. Try offering this National Kidney Foundation (NKF) brochure to your patient a week beforehand so it gives them a chance to read up on the modalities before you sit down with them.

The “2019 Project Attestation and Root Cause Analysis” was due January 7^th for all project participants. Please use the link in your project notification.

Project kickoff calls with full instructions will be available online; please use the link in your project notification.

The national LAN calls will be starting again in January! Project participants who are required to attend will receive an email invitation. The January schedule is:

-BSI LAN call January 8

-Home LAN call January 15

-Transplant LAN call January 22

“Performance Score Certificates Available Now” Payment Year (PY) 201 9 ESRD QIP Final Performance Score Reports (PSR) and Performance Score Certificates (PSC) were released in December, which show your facility’s performance during 2017.

Remember you are required to post both the English and Spanish versions in a prominent patient area within fifteen (15) business days. Both versions must be signed by the Facility Medical Director . If you have any questions, please contact the ESRD QIP Team at [email protected].
