So that we can efficiently diagnose and treat our mutual patients and keep their costs at a minimum, when you refer your patient to our office we ask that you send the most recent FMX or Pano, bitewings and PAXs, photos and if possible CBCT scans. In addition to x-rays, we also ask that you fill out a REFERRAL FORM and email that to us along with any other attachments that you feel are important for us to properly diagnose and treat your patient.
