"IN THE CHAIR...DENTAL IMPLANTS!"
Your office's implant preference is key! Complete this FORM and submit your office's preference ASAP!
Scheduling for a Dental Implant with EVP!
  1. Complete referral form (include fmx/pano and recent PA of site)
  2. Patient schedules Implant Consultation with EVP.
  3. Patient arrives for consult.
  4. Surgery is scheduled.
  5. Exam letter is emailed to your office.
  6. Surgery is completed.
  7. Surgical letter is emailed to your office indicating what size and brand of implant was placed(be sure implant preference is notated on referral).
  8. 3 month implant check is completed and when implant is ready to be restored, an Implant Memo is emailed to your office (typically 3-4 months from surgery).

Referring a patient to EVP...
ONLINE REFERRAL FORMS ARE HERE!
One of our main goals is to provide outstanding communication with your office from the moment your patient calls to schedule their initial appointment for any of the following:

Limited Exams
Comprehensive Evaluations
Implant Consultations
Hybrid Consultations

So that we can efficiently diagnose and treat our mutual patients and keep their costs at a minimum, when you refer your patient to our office we ask that you send the most recent FMX or Pano, bitewings and PAXs, photos and if possible CBCT scans. In addition to x-rays, we also ask that you fill out a REFERRAL FORM and email that to us along with any other attachments that you feel are important for us to properly diagnose and treat your patient.
INSURANCE PLANS WE PARTICIPATE IN:

The following is a listing of the insurance companies that we are “in network” with:

Cigna PPO 
Delta Dental PPO
Delta Dental Patient Direct & Cigna Discount Plans
Careington Network

We gladly file claims for all other indemnity and PPO plans that we are not in network with and will accept assignment of benefits where it is available. Patients are responsible for any difference of fees when seeing an out of network provider. 

We also offer no interest financing through:
Alphaeon
Care Credit
Lending Club 
(Patients must qualify. We do not offer in-house financing.)
