From your School Nurse:

Please be sure to send in your updated shot records after getting your required vaccines. All vaccines must be up to date to stay in school.

Pre K and KDG must meet state requirements for vaccines. If your child can not take vaccines, a letter of exemption must be on file which requires a doctor’s signature.

Pre-K must have at least one of the following:

DtaP

Polio

Hep B

MMR

Varicella

KDG must have all vaccines in these:

DtaP 4 doses Both Dtap and Polio last dose must be after

Polio 3 doses 4th birthday

Hep B 3 or 4 doses

MMR 2 doses

Varicella 2 doses
