J9071 Injection Cyclophosphamide, (Auromedics), 5 mg is available for all sites of care

C9087 Injection Cyclophosphamide, (Auromedics), 10 mg will be deactivated by CMS

J9071 is a unique HCPCS code for Auromedics' ready to dilute Cyclophoshamide and should not be used for billing other formulations.

Please note the change of the HCPCS code number, as well as a change in code descriptor dosage from 10 mg to 5 mg
