If an attachment is submitted to a previously submitted claim to make a final decision, it's important to remember the following when submitting the attachment through our web portal.
- If there is no previous adjustment made to a claim (the claim number would not have an A1 appended to the claim number), then the attachment should be attached to the original claim number.
- If there was a previous adjustment made to a claim (the claim number would have an A1, A2, A3, etc. appended to the claim number), then the attachment should be attached to the last adjusted claim. It's very important to make sure you attach your attachment to the last adjustment made to a claim.
Attachments should always be attached to the latest denial or adverse action. If the attachment is submitted to an earlier version of the claim, our system will not recognize the attachment to generate a new claim. For example, if A1 already exists on the claim number, the system will not recognize the new attachment to generate a new claim (an A2 claim will not be generated).
