Home and Community Care in Elgin
Do you know you have a Home and Community Care Coordinator assigned to your practice? If you haven't yet met yours, they can be a great resource to you and your practice population. South West LHIN MRI Forms

A reminder that the SW LHIN has moved to a  SINGLE MRI REQUISITION  for all MRI's in our LHIN.

For MRIs of spine or knee, the Choosing Wisely checklist (also available on website) must also be completed and sent as of Jan 30, 2018. For all other MRIs this is not needed.

Using older requisitions will likely lead to test being declined!
Remove old paper forms from your office and de-activate old custom forms from your EMRs!

OCFP Awards

We have lots of great Family Physicians in Elgin! Let's nominate one of our colleagues for recognition by the Ontario College of Family Physicians. Forward names to Kellie or Melissa and we will help submit a nomination.
