Home and Community Care Coordinators in Elgin have recently been realigned to match your practice population! Please take time to meet with your Care Coordinator to discuss any patients you may have concerns about and le t them kno w the best way to communicate with you. Hopefully this will be a more efficient system as you will have one main Care Coordinator for most of your patients. (Some patients require specialized care by a different team). If you do not know who your Care Coordinator is, contact Rebecca Sutcliffe [email protected]

The Health Links Team has identified top ER users and may be reaching out to see how we can help support these patients and you as their primary care provider with a Coordinated Care Plan. If you have questions about Coordinated Care Planning , contact Diane Arsinijevic [email protected]

Your patients, 15 years old and older, can now access free psychotherapy services

Bounce Back Ontario https://bouncebackontario.ca is now offering free skill-building program designed to help adults and youth 15+ manage symptoms of depression and anxiety. This is for mild to moderate depression and anxiety (ie. PHQ 9 score of < 21.)

MAID Forms can be found on the SWPCA Website. Julie can help arrange mentorship opportunities if it is something you are interested in or help you access this care for a patient, whether you want to be directly involved or not.

London's Ambulatory Psychiatry

e-referral process on hold after your feedback!

On January 15, 2018, London Health Sciences Centre (LHSC) in collaboration with St. Joseph’s Health Care London (St. Joseph’s) launched the Centralized Access Point (CAP) and e-referral form for Adult Ambulatory Mental Health Care Services. Extensive feedback post-launch from primary care practitioners identified that the e-referral form and platform did not align to the primary care work flow. As a result, LHSC and St. Joseph’s have agreed to eliminate the CAP e-referral process and return to fax as the primary method of accepting referrals . Additionally, in partnership with primary care the referral form will be redesigned to a simple, writable and printable PDF document. In addition, based on feedback from Primary Care, our goal is to have this form converted into usable formats for your electronic medical record (EMR). Until the referral form redesign is complete, the CAP e-referral process will remain operational. The CAP office will continue to accept referrals submitted using the current referral form via e-referral or fax to 519-667-6685 . Please access the e-referral form here. Additionally in the interim, the CAP office will continue to accept older versions of the referral forms for the Adult Ambulatory Mental Health Care programs via fax to the number noted above. Thanks to Dr. Gord Schacter, Dr. Jillian Toogood, and the St. Joseph's and LHSC teams for their collaboration on this.
