6. Neighborhood Health Homes: Care Networks Built on Integrated Primary Care and Community Accountability
📅 October 29, 2026 | 🕒 11:00-12:00pm | 💻 Virtual
Ontario's vision for connected primary care depends on more than expanding access. It requires strong relationships between primary care organizations, Ontario Health Teams, community partners, specialists and hospitals that are built around the needs of local communities and the full population of people within them.
Join leaders from Southlake Academic Family Health Team, the Northern York South Simcoe Ontario Health Team, and Southlake Health as they share how they are building a neighbourhood-based model of integrated care across 6 unique communities in the Northern York South Simcoe region.
Rather than creating new structures, their approach strengthens the relationships that already exist, connecting primary care, community services, specialists and hospital care into a coordinated network that will support providers to create better experiences for all local residents via guaranteed attachment and improved access to primary care.
Participants will hear practical lessons on developing the partnerships, governance and shared approaches needed to create a strong foundation for integrated care while maintaining the unique strengths of each organization.
Whether your community is beginning this work or looking to strengthen existing partnerships, this webinar will provide practical ideas that can be adapted to your local context.

10. Mentorship Program for Nurse Practitioners
RNAO has launched a newly revised Nurse Practitioner Mentorship Program, in collaboration with the Nurse Practitioner Interest Group, designed to support NPs as they navigate their professional journeys and strengthen our nursing community across Ontario.
Whether you are a new NP beginning practice or an experienced NP looking to support and inspire the next generation of colleagues, RNAO invite you to become part of this rewarding initiative.
For New NPs
Transitioning into NP practice is an exciting milestone. It can also bring new challenges as you develop confidence in your clinical decision-making, leadership abilities and professional identity. Through this program, you'll be matched with an experienced NP mentor who can offer guidance, encouragement and practical insights as you establish yourself in practice.
For Experienced NPs
Your knowledge, experience and leadership can make a meaningful difference in another NP's transition to practice. Serving as a mentor is an opportunity to give back to the profession, support emerging colleagues and contribute to the future of high-quality, person-centred care in Ontario.
The program includes:
- Monthly mentor-mentee meetings
- Bi-monthly peer support and community of practice sessions
- Orientation and resources to support successful mentorship relationships
- Opportunities to build meaningful professional connections with NPs across Ontario
Mentorship is more than sharing expertise. It is about creating supportive relationships that foster confidence, professional growth, resilience and a stronger nursing profession.
As RNAO continues to build and strengthen mentorship opportunities, they also hope to expand this work in the future by developing mentorship programs for new RNs entering practice, helping support successful transitions across the nursing profession.
Whether you are seeking guidance or ready to share your experience, we encourage you to join in creating a supportive community where nurses learn from one another and succeed together.
